What the Storage Request Form Is and when to use it

A Storage Request Form documents a party's instruction to place, transfer, or retain physical or electronic records or goods in a designated storage facility or records archive. It identifies the requester, the items or record sets, storage period, access permissions, special handling or security needs, and any associated fees or insurance. Organizations use the form to create an auditable chain of custody, to establish liability and access rules, and to coordinate receiving, indexing, and disposition activities across departments or third‑party providers.

Why a clear Storage Request Form matters

A well‑structured form reduces disputes about custody and access, supports regulatory retention and privacy obligations, and creates a single authoritative record for billing, chain of custody, and disposition decisions.

Why a clear Storage Request Form matters

Who typically completes and receives this form

Listing roles and contact details on the form prevents processing delays and ensures correct chargeback and access control.

  • Records managers and facilities staff who coordinate intake, indexing, and secure storage access for organizational records.
  • Departmental requesters (legal, HR, finance) who authorize retention or retrieval of specific files or assets.
  • Third‑party storage vendors or fulfillment teams that need legal instructions and billing details to accept or release items.

Essential sections to include in a professional Storage Request Form

Include concise sections that establish identity, item detail, handling, access, duration, and signatures to make the request complete and enforceable.

Requested start and end dates, renewal options, insurance requirements, and unit type (pallet, box, digital archive) to set expectations.

Access & Authorization

Names and roles of authorized requesters and delegates, access windows, and required ID or credentials for pickup or electronic retrieval.

Special Handling

Temperature control, HIPAA/FERPA restrictions, hazardous materials, or chain‑of‑custody instructions that affect storage workflows and compliance.

Signatures

Signature blocks, printed name, title, date, and witness or notary lines where required to validate authority and acceptance of terms.

Common mistakes to avoid when preparing this form

  • Using informal identifiers (e.g., 'old files') instead of precise box or record numbers, which causes misplacement and retrieval delays.
  • Failing to list authorized signers clearly, resulting in denied access or disputes when third parties attempt to pick up items.
  • Neglecting to note confidentiality or regulatory status (HIPAA, FERPA), which can expose the organization to compliance violations during storage or transit.
  • Forgetting to attach or reference insurance and handling instructions for valuable or fragile items, leading to liability gaps on damage claims.

Typical signers and approvers for a Storage Request Form
