STUDENT CLASS REPRESENTATIVE AGREEMENT

This Student Class Representative Agreement (the Agreement) is entered into as of Date: between Student Name: (the "Representative") and Institution Name: with principal address: (the "Institution").

RECITALS

WHEREAS, the Institution organizes classes and desires a designated Student Class Representative for the class identified below to facilitate communication between students and Institution officials; and

WHEREAS, the Representative is a currently enrolled student eligible under applicable Institution policies and has agreed to serve as the Representative for the class identified as: Class Designation: ;

WHEREAS, the parties desire to set forth in writing the duties, term, standards of conduct, and administrative procedures applicable to the Representative.

NOW, THEREFORE

NOW, THEREFORE, in consideration of the mutual covenants set forth herein, the parties agree as follows:

1. APPOINTMENT; TERM

1.1 Appointment. The Institution hereby appoints and the Representative accepts appointment as the Student Class Representative for the class identified in this Agreement. The Representative shall act in the best interests of the student body of the class and perform the duties set forth in Section 2.

1.2 Term. The term of this Agreement commences on Start Date: and continues until End Date: unless earlier terminated in accordance with Section 3.

2. DUTIES AND RESPONSIBILITIES

2.1 Core Duties. The Representative shall:

a) Serve as the primary point of liaison between enrolled students in the class and designated Institution officials; b) collect and communicate student concerns, suggestions, and feedback in an accurate and timely manner; c) convene and document class meetings or feedback sessions as needed; and d) assist with dissemination of official notices to classmates as authorized by the Institution.

2.2 Reporting. The Representative shall prepare written summaries of issues raised by students and provide such summaries to the Faculty Advisor or designated administrator at the following frequency: . Summaries must be accurate, non-defamatory and must not disclose personal student information except as authorized.

2.3 Conduct. The Representative shall perform duties professionally, maintain impartiality when representing classmates, and comply with all applicable Institution policies and codes of conduct.

3. TERMINATION; REMOVAL

3.1 Termination for Convenience. Either party may terminate this Agreement for any reason upon written notice delivered in accordance with Section 11. The effective date of termination shall be thirty (30) days after receipt of such notice, unless otherwise agreed in writing.

3.2 Removal for Cause. The Institution may remove the Representative immediately for cause, including material breach of this Agreement, violation of Institution policies, misrepresentation of student views, negligence in performance of duties, or conduct that materially disrupts the educational process. Removal shall be evidenced by written notice to the Representative.

4. CONFIDENTIALITY; PERSONAL DATA

4.1 Confidential Information. The Representative will not disclose confidential information acquired in the course of performing duties, including but not limited to student records, disciplinary matters, or personnel information, except as required by law or with prior written authorization of the Institution.

4.2 Data Handling. The Representative shall handle personal data in compliance with applicable privacy laws and Institution policies, only collecting or transmitting personal information that is necessary and authorized for legitimate class representative functions.

5. COMPENSATION AND EXPENSES

5.1 Compensation. The Representative acknowledges that service is primarily voluntary. Any stipend, honorarium or other compensation must be expressly authorized in writing by the Institution. Will the Representative receive compensation or stipend? Yes No

5.2 Expenses. The Institution will reimburse only pre-approved, reasonable expenses directly incurred in performance of duties upon submission of receipts and in accordance with Institution procedures.

6.1 Disclosure. The Representative shall promptly disclose any actual or potential conflict of interest that could impair the Representative's impartial performance of duties.

6.2 Certification. By signing this Agreement the Representative certifies that they are an enrolled student in good standing eligible to serve, and that the information provided in this Agreement is true and complete to the best of their knowledge. If eligibility ceases, the Representative must promptly notify the Institution.

7. RECORDS; DOCUMENTATION

7.1 Records. The Representative shall maintain written records of meetings, student concerns, and communications made in the course of performing duties and shall provide such records to the Institution upon request.

7.2 Ownership. Records created exclusively for official Institution purposes are institutional records. Personal notes retained by the Representative are not institutional records unless delivered to or adopted by the Institution.

8. NOTICES

Notices must be in writing and shall be delivered personally, by recognized courier, or by mail to the addresses provided above, and shall be deemed given on the date of receipt.

9. AMENDMENTS; WAIVER

This Agreement may be amended only by a written instrument signed by both parties. Failure to enforce any provision shall not constitute a waiver of future enforcement.

10. GOVERNING LAW; ENTIRE AGREEMENT; SEVERABILITY; COUNTERPARTS

10.1 Governing Law. This Agreement shall be governed by and construed in accordance with the laws of the state or jurisdiction in which the Institution is located, without regard to choice of law principles.

10.2 Entire Agreement. This document constitutes the entire agreement between the parties with respect to the subject matter and supersedes all prior oral or written agreements and understandings.

10.3 Severability. If any provision of this Agreement is determined to be invalid or unenforceable, such provision shall be severed and the remaining provisions shall continue in full force and effect.

10.4 Counterparts. This Agreement may be executed in counterparts, each of which shall be deemed an original and all of which together shall constitute one instrument.
