What the Supplier Discovery Questionnaire Is and When It’s Used

The Supplier Discovery Questionnaire is a standardized form used to collect background, compliance, insurance, financial, and capability information from potential vendors before procurement or onboarding. It typically gathers company identity, ownership, key contacts, certifications, insurance limits, regulatory registrations, conflict-of-interest disclosures, and sample work references. Organizations use the questionnaire early in vendor selection to screen suppliers, verify minimum requirements, and document due diligence. Completed questionnaires are often retained with procurement records and may be submitted electronically and signed to create an auditable record of supplier representations.

Why a Structured Questionnaire Matters for Procurement

A standardized Supplier Discovery Questionnaire centralizes vendor data, reduces duplicate requests, and documents due diligence. It speeds evaluation, supports compliance checks (insurance, sanctions, licensing), and creates a written record of supplier assertions that legal, finance, and procurement teams can review consistently.

Why a Structured Questionnaire Matters for Procurement

Which Teams Typically Complete or Request This Questionnaire

Procurement, vendor management, legal, and finance teams commonly issue the Supplier Discovery Questionnaire as part of supplier onboarding and prequalification.

  • Procurement and sourcing teams: standardize vendor selection and compare suppliers across cost, capability, and compliance
  • Legal and compliance: verify regulatory registrations, sanctions screening, and contract-enforceable representations
  • Finance and audit: confirm tax IDs, insurance coverage, and financial stability for payment and risk assessment

Completed questionnaires help subject-matter experts validate claims, reduce onboarding time, and create an auditable file for future contract and compliance reviews.

Core Sections to Include in a Professional Supplier Discovery Questionnaire

A complete questionnaire combines identification, compliance, capability, financial, references, and signature sections so reviewers can quickly assess fit and risk before entering contracts.

Company Identity

Legal business name, DBA, entity type, state/country of formation, employer identification number (EIN), and principal office address to confirm legal standing and contracting party.

Ownership and Control

Certifications and Licenses

Professional licenses, industry certifications, minority-owned or small-business certifications, and license numbers with expiration dates for compliance verification.

Insurance and Limits

Insurance carriers, policy numbers, coverage types, and limits (general liability, professional liability, workers' comp) to validate contractual indemnity and minimum coverage requirements.

Financial and Performance Data

Recent audited or unaudited financial statements, bank references, annual revenue bands, and performance metrics to assess supplier stability and capacity.

References and Samples

Customer references, project descriptions, delivery timelines, and sample deliverables to confirm relevant experience and on-time performance history.
