What the Support Time Request Form Is and When It’s Used

The Support Time Request Form is an administrative document for requesting, recording, and authorizing technical or professional support hours. It typically captures requester identity, service description, ticket or project reference, start and end times, total hours, billing codes, approval signatures, and attachments that substantiate the work. Organizations use this form to standardize intake, reduce billing disputes, link time to projects, and create auditable records. The form can be implemented as a fillable PDF, web form, or e-submission and may be signed electronically in accordance with ESIGN and UETA requirements for enforceability.

Why a Standardized Support Time Request Form Matters

A consistent form reduces approval delays, prevents billing errors, and produces a verifiable audit trail for accounting and compliance. It clarifies scope, responsibility, and charges so project managers, approvers, and finance teams can reconcile time and invoice clients with supporting documentation.

Why a Standardized Support Time Request Form Matters

Typical Users and Roles

Common users include people who request services, the managers who approve them, and the finance staff who record and bill time.

  • Requesters: Staff or contractors who record time and provide ticket or project references for verification.
  • Approvers: Supervisors or project managers who review service descriptions and authorize billable hours.
  • Finance / Payroll: Teams that validate codes, reconcile charges, and process invoices or payroll adjustments.

Coordinating these roles on a single standardized form reduces back-and-forth, accelerates approvals, and ensures consistent accounting treatment across departments.

Essential Elements to Include on the Form

Include fields that support identification, scope, time capture, authorization, billing, and evidence so the form is useful for operations, audits, and client billing.

Requester Details

Full legal name, department, contact info, and client or account number. Accurate identity prevents misbilling and supports matching to HR and billing records.

Service Description

Concise description of tasks performed with a ticket or project reference. Clear scope lets approvers and clients verify the work before billing.

Time Entries

Start/end times, total hours, and time category (billable or non-billable). Use the organization’s accepted format and rounding policy for consistency.

Approval Block

Signature fields, approver name, and approval date. Include electronic signature metadata when the form is executed online to create an audit trail.

Billing Fields

Cost center, GL code, hourly rate or billing code, and invoice reference. These fields ensure accurate chargeback and automated posting to accounting systems.

Attachments

Ability to attach logs, screenshots, change orders, or time-tracking exports. Supporting documents substantiate charges during audits and client reviews.

Four Steps to Submit and Process a Request

Complete and route the Support Time Request Form in four stages to ensure accurate capture, authorization, and posting to billing or payroll systems.

  • 01
Fill Form: Enter requester, service details, and time entries accurately.
  • 02
Attach Evidence: Add logs, screenshots, or ticket links supporting the work.
  • 03
Request Approval: Send to the designated approver with authorization metadata and due date.
  • 04
Submit to Billing: Forward approved form to finance for invoicing and GL reconciliation.

How to Configure an Online Form and Approval Workflow

Set up online form behavior and automated routing to reduce manual steps and enforce data validation before submission.

Field Configuration
Prefill Requester Info Auto-populate from HR or corporate directory to reduce manual entry
Validation Rules Require MM/DD/YYYY for dates and HH:MM for time entries
Conditional Fields Show billing fields only when billable is selected
Approval Routing Route by department, project code, or manager hierarchy

Where Completed Forms Go and Who Sees Them

A clear routing path ensures each stakeholder receives the form they need for review, approval, or recordkeeping.

  • Requester: Initiates the request and receives a submission confirmation
  • Approver: Reviews details, approves or returns for clarification
  • Finance: Receives approved forms to post charges and prepare invoices
  • Records: Stores signed copies per retention policy and audit requirements

Technical Requirements for Digital Completion and Signing

Digital completion and eSignature require a platform that supports stable file formats, secure access controls, and a comprehensive audit trail.

  • File Formats: PDF, DOCX, and fillable form support
  • Integrations: Connectors to HR, ERP, and ticketing systems
  • Authentication: Email, SMS, SSO, or stronger MFA options

Typical Timelines and Cutoffs to Observe

Set timelines for submission, approval, and billing cutoffs and communicate them to requesters to prevent missed charges and delayed payroll.

Submission Deadline:

Submit within five business days of work completion

Approval SLA:

Approvers respond within three business days of receipt

Billing Cutoff:

Forms received before month-end are processed that month

Dispute Window:

Clients may dispute charges within 30 days of invoice

Record Update:

Allowed corrections accepted within 15 days of submission

Common Mistakes to Avoid

  • Incomplete time entries or missing AM/PM details create ambiguous hours and force manual follow-up and corrections before approval.
  • Vague service descriptions without ticket references make verification difficult for approvers and auditors, increasing dispute risk.
  • Incorrect project codes or account numbers cause misallocation of costs and additional reconciliation work for finance teams.
  • Uploading unsupported file formats or oversized attachments can block transmission and leave supporting evidence unavailable during review.

Risks of Inaccurate or Untimely Forms

Billing Rejection: Charges denied by client
Payroll Delay: Staff payments postponed
Audit Findings: Financial or compliance citations
Backup Withholding: Tax withholding risk for bad TINs
Contract Dispute: Unauthorized work claimed
Data Exposure: Incorrect access risks PHI
