What the Tax Authorization 4506C Is and when it’s used

The Tax Authorization 4506C is the IRS form used to authorize an authorized third-party requestor to obtain a taxpayer's tax return transcript through the Income Verification Express Service (IVES). It documents taxpayer consent for a specific requestor to access transcripts for defined tax years or types of return information. Lenders, servicers, and other IVES participants use the form during underwriting, verification, or compliance reviews. The form collects taxpayer identifiers, the requester’s details, requested transcript types, and a taxpayer signature or authorized representative signature to complete the authorization process.

Why confirming authorization matters for transcript requests

Form 4506C establishes explicit taxpayer consent required by the IRS before an authorized third party may receive tax transcripts; it documents the scope and duration of permission and reduces downstream compliance risk.

Why confirming authorization matters for transcript requests

Typical users and who completes the form

The form is completed when a taxpayer or authorized representative gives permission for a named requester to obtain tax transcripts.

  • Mortgage lenders and loan processors who participate in IVES and need tax verification for underwriting.
  • Third-party verification companies or settlement agents obtaining transcripts on behalf of a lender or servicer.
  • Taxpayers or authorized representatives who sign to grant an identified requester access to specific tax year transcripts.

Copies are retained by the requester and often stored in the loan or verification file to demonstrate lawful access.

Core elements included on a professional 4506C

A compliant 4506C clearly identifies the taxpayer, the requesting party, the precise records requested, the tax periods covered, the signature block and the date; each component supports a valid transcript release to an authorized recipient.

Taxpayer identity

Full legal name, SSN or ITIN, and current address to match IRS records and avoid misrouting requests.

Requesting party

Name, address, and IVES participant identifier (if applicable) for the entity authorized to receive transcripts.

Transcript scope

Specific years and transcript types (e.g., Form 1040 transcript, wage transcript) to limit data release to only requested items.

Signature and date

Original signature of taxpayer or authorized representative and a clear date to record consent timing.

Representative info

If signed by an agent, include agent name, capacity, and documentation of authority (power of attorney or written authorization).

Instructions block

Lines that specify delivery method (mail, fax, secure electronic delivery) and any requester submission details required by the IRS.

Step-by-step: completing and submitting a 4506C

Follow these sequential steps to complete the form, confirm authorization, and deliver the request to the appropriate recipient.

  • 01
Prepare taxpayer data: Gather full name, SSN/ITIN, address, and tax years to request.
  • 02
Complete requester details: Enter the authorized requester's name, address, and participant identifier if applicable.
  • 03
Sign and date: Taxpayer or authorized representative signs and dates the form in MM/DD/YYYY format.
  • 04
Submit to requester: Provide the signed form to the requester for their submission to the IRS via IVES or other approved channel.

Configuring an online 4506C workflow

Typical online configuration settings help ensure consistent capture of required fields and secure delivery to authorized requestors.

Field Configuration
Required fields Make name, SSN/TIN, address, years, transcript type, and signature required
Authentication Use email + SMS code or stronger signer authentication for identity proofing
Conditional fields Show agent/power-of-attorney fields only when representative signing is selected
Delivery method Route signed forms to secure document repository or IVES transmission queue

Where completed 4506C forms are sent

After signature, the requester submits the form to the IRS through the authorized channel or retains it for IVES transmission per IRS procedures.

  • Deliver to requester: Provide the executed form to the named requester for their records or submission.
  • IVES transmission: Authorized IVES participants submit requests electronically to the IRS IVES system.
  • Secure storage: Store the signed form in the verification file or secure document repository.
  • Provide copies: Requester retains copies to demonstrate taxpayer consent during audits or reviews.

Digital signing and e‑submission considerations

Use platform controls that preserve intent, attribution, and an immutable audit trail when completing 4506C electronically.

  • Authentication options: Email, SMS code, or stronger methods
  • Audit trail: Capture IP, timestamp, and actions
  • Encryption: TLS in transit; AES-256 at rest

Timelines and processing expectations for transcript requests

IRS processing times vary by delivery method and volume; plan for potential delays and preserve signed authorizations in the file until the transcript delivery completes.

Immediate transmission:

IVES electronic requests often return transcripts same day or within 1–3 business days

Paper processing:

Paper-based requests or mail submissions can take multiple weeks

Record retention:

Keep the signed authorization until audit risk passes and retention requirements elapse

Follow-up re-submission:

If IRS rejects the request, correct and resubmit promptly to avoid underwriting delays

Customer notifications:

Notify taxpayers if additional verification or identity proofing is required

Key risks and potential penalties from errors

Unauthorized disclosure: Civil/criminal liability risk
Processing delay: Loan or verification delays
Incorrect TIN: Backup withholding or rejections
False authorization: Fraud allegations and remediation costs
Intentional misuse: Statutory penalties under tax law
Incomplete form: IRS denial or resubmission required

Common mistakes to avoid when preparing a 4506C

  • Leaving the SSN or ITIN field incomplete or mismatched to IRS records, which commonly causes request rejections and delays.
  • Failing to list exact tax years or transcript types; vague or overly broad requests may be rejected by the IRS.
  • Allowing an agent to sign without attaching proof of authorization or power of attorney, creating challenges during verification or audits.
  • Using unsecured delivery channels for signed forms, increasing the risk of interception or unauthorized disclosure of tax data.

Security and compliance controls that apply to electronic 4506C handling

Encryption in transit: TLS 1.2/1.3
Encryption at rest: AES-256
SOC 2 Type II: Certification available
HIPAA support: BAA required
21 CFR Part 11: Compliant options available
ESIGN / UETA: E-signature legal compliance

Real-world examples of transcript authorization workflows

Below are two brief examples of how organizations collect and use 4506C forms in operational workflows.

Optica Ventures (COO)

Optica integrates a digital authorization step into loan intake to minimize paper handling and audit friction.

  • They route signed 4506C forms to a secure queue for IVES transmission.
  • This reduced manual scanning and produced a consistent audit trail for each transcript request while ensuring the taxpayer’s consent document was retained with the loan file.

Fertility Centers of Illinois (Founder)

A health services organization adopted secure e-signing to obtain patient consent for limited financial verification.

  • They used agent authorization where family members signed on behalf of patients.
  • The process preserved privacy controls, required documented proof of representative authority, and maintained an auditable record of consent linked to each verification request.

Best practices for accurate and efficient completion

These practices reduce rework, protect taxpayer data, and help ensure timely transcript delivery.

Verify identity before signing
Confirm the taxpayer’s name and SSN/ITIN match IRS records; use multi-factor authentication or knowledge-based checks when possible to reduce rejected requests.
Limit requested scope
Request only necessary tax years and transcript types to reduce exposure and speed IRS processing; overly broad requests can trigger additional review.
Document representative authority
When an agent signs, attach a power of attorney or written authorization to the file to avoid later disputes or evidence gaps in audits.
Maintain secure storage
Store signed forms in encrypted repositories with role-based access and audit logs to meet retention and privacy requirements and to support compliance reviews.
