The undersigned (the "Client") engages the tax preparer (the "Preparer") to prepare the Client's federal and applicable state income tax return(s) for the tax year: . 2. Scope of Services

3. Fees and Payment

4. Client Responsibilities

The Client agrees to provide complete and accurate information necessary for the preparation of returns, to retain records supporting items reported on the tax returns, and to promptly respond to Preparer requests. The Client is responsible for the completeness and accuracy of all information provided.

5. Confidentiality and Use of Information

The Preparer will maintain the confidentiality of Client information except as authorized by the Client or required by law. The Client authorizes the Preparer to disclose return information to third parties only as necessary to complete the engagement or as otherwise authorized herein.

6. Authorization to Discuss Tax Matters

The Client authorizes the Preparer to discuss and receive tax information from designated third parties (financial institutions, payroll providers, prior preparers) to the extent necessary to prepare the Client's returns.

7. Taxpayer Identification

Provide the Client's taxpayer identification below. Complete either the Social Security Number (SSN) or Employer Identification Number (EIN) field as applicable.

8. Limitation of Liability and Record Retention

The Preparer's liability for errors, omissions or other claims arising from this engagement is limited to the amount of fees paid for the specific service giving rise to the claim, except where prohibited by law. The Client agrees to retain records and supporting documentation for the period required by law; the Preparer may retain copies in accordance with professional retention practices, but is not obligated to maintain original documents.

9. Termination

Either party may terminate this engagement upon written notice. The Client remains responsible for payment of fees and expenses incurred up to the effective date of termination, and for any additional costs reasonably required to transition files.

10. Certification and Perjury Statement

By signing below, the Client certifies, under penalties of perjury, the following:

  1. All information provided to the Preparer for the preparation of tax returns is true, correct, and complete to the best of the Client's knowledge.
  2. The Client will retain records, receipts, and other supporting documents sufficient to substantiate items of income, deductions, credits and other information reported on tax returns.
  3. The Client understands that the Preparer will rely exclusively upon documents and information provided by the Client and that the Preparer is not responsible for detecting fraud or misrepresentation by third parties.
  4. The Client authorizes the Preparer to discuss and disclose return information to third parties only as necessary to fulfill this engagement or as otherwise authorized in writing by the Client.
  5. The Client agrees to cooperate with any subsequent inquiries or examinations by taxing authorities and to provide additional information as required.
  6. The Client acknowledges that the Preparer does not guarantee results and is not responsible for penalties, interest, or additions to tax assessed by taxing authorities due to inaccurate or incomplete information provided by the Client.
