Enrollment Details

The taxpayer requests enrollment for the following tax periods and return types. The tax preparer is authorized to prepare, sign where permitted, and electronically file the returns indicated below on behalf of the taxpayer.

Representation and Authorization

By signing this Agreement, the taxpayer authorizes the designated tax preparer and its agents to: (a) prepare and file the indicated returns; (b) sign returns on the taxpayer's behalf where permitted by law and by separate power of attorney or written authorization; (c) receive and transmit tax information necessary to complete filings; and (d) communicate with tax authorities regarding the enrolled returns.

Fees, Billing and Payment Authorization

The taxpayer agrees to pay fees as separately disclosed by the service provider. Unless otherwise agreed in writing, the taxpayer authorizes collection of fees and any authorized penalties or interest from the payment method provided below.

Confidentiality and Data Security

The service provider will safeguard taxpayer information in accordance with commercially reasonable security practices and applicable law. The taxpayer acknowledges that transmission of tax information electronically carries inherent risks and consents to the transmission and storage of such information by the service provider for the purposes described in this Agreement.

Consent to Electronic Delivery and Signature

The taxpayer consents to receive disclosures, notifications, tax forms, and other communications electronically and to use an electronic signature that will have the same force and effect as a handwritten signature where permitted by law.

Taxpayer Certifications (Perjury and Accuracy)

The taxpayer certifies, under penalties of perjury, that the information provided in this Agreement and any attachments is true, correct, and complete to the best of the taxpayer's knowledge. The taxpayer further acknowledges and agrees to the following numbered certifications:

  1. All taxpayer identifying information provided in this Agreement, including the Taxpayer Identification Number, is accurate and belongs to the taxpayer for whom the returns will be filed.
  2. The taxpayer authorizes the service provider to prepare, sign where applicable, and electronically file the enrolled returns and to reconcile any electronic filing rejections or notices related to those returns.
  3. The taxpayer will provide accurate and complete documentation and information required to prepare the enrolled returns and will timely inform the service provider of any changes that affect the returns.
  4. The taxpayer acknowledges that the service provider is not guarantying any particular tax outcome and that the taxpayer retains legal responsibility for the accuracy of the information submitted to tax authorities.
  5. The taxpayer authorizes disclosure of taxpayer information to third parties as necessary to prepare and file the enrolled returns, to respond to tax authority inquiries, and to collect fees as authorized herein.

The taxpayer certifies that the Taxpayer Identification Number provided on this form is correct and that the taxpayer is not subject to backup withholding unless notified by tax authorities. If the taxpayer is subject to backup withholding, the taxpayer must notify the service provider immediately.
