What the Team Building Activity Form Is and When It’s Used

A Team Building Activity Form documents the logistics, approvals, risks, and participant information for a work-sponsored group event. Typical content includes event date and location, participant names and contact information, emergency contacts, dietary needs, transportation arrangements, cost allocation, manager approval, and a liability and media release or waiver. Organizations use the form to centralize planning details, confirm participant consent and medical needs, support budgeting, and create a record for risk management and insurance purposes. The form can be completed on paper or electronically and is commonly routed for manager sign-off and HR review.

Why a Formal Form Matters for Team Events

Using a formal Team Building Activity Form reduces ambiguity about responsibilities, documents participant health and dietary needs, and creates a clear audit trail for approvals and costs. It helps employers meet duty-of-care expectations and supports insurance or incident response if problems occur.

Why a Formal Form Matters for Team Events

Who Typically Completes and Reviews This Form

Completed forms are retained with HR or departmental records and shared with safety or legal teams when required.

  • Event Coordinator — Prepares itinerary, collects participant data, and coordinates vendors and transportation approvals.
  • Human Resources — Reviews waivers, medical disclosures, and consent language for compliance and records retention.
  • Managers / Supervisors — Approve budgets, verify participant eligibility, and confirm staffing coverage during events.

Vendor Contracts

Insurance Certificates

Include proof of vendor liability insurance and any additional insured endorsements; these documents protect the organization in the event of a claim.

Medical Release Forms

For higher-risk activities, attach participant medical release or physician clearance forms as required by organizational policy to document fitness to participate.

Budget Spreadsheets

Common Mistakes When Preparing a Team Building Activity Form

  • Incomplete emergency contact or medical information. Missing details delay response during incidents and can increase organizational liability.
  • No manager or budget approval before commitments. This causes last-minute cancellations, cost overruns, or lack of expense coding for finance.
  • Failing to attach vendor insurance or contracts. Without insurance certificates the organization may be exposed if a vendor-related incident occurs.
  • Using ambiguous waiver language. Vague or overly broad waivers may be unenforceable and create gaps in risk transfer.

Risks and Consequences of an Incomplete or Improper Form

Liability Exposure: Civil claims or higher insurance payouts
Medical Risk: Missed allergy or condition information
Budget Overruns: Unapproved spending and accounting issues
Policy Violations: Noncompliance with org rules
Enforceability: Waivers may be void if signatures invalid
Recordkeeping: Insufficient records can impede claims defense
