Waiver, Release, and Acknowledgments

In consideration of being permitted to participate in Team activities, Participant or Participant's parent/guardian, on behalf of Participant, hereby acknowledges, warrants and agrees as follows:

1. Assumption of Risk: Participant understands that participation in athletic activities involves risk of injury, illness, or death, and assumes full responsibility for such risks arising from participation, whether caused by negligence of the Organization or otherwise.

2. Release and Indemnity: Participant, and Participant's parent/guardian if applicable, release, waive and covenant not to sue the Organization, its directors, officers, coaches, volunteers and agents from any and all claims, liabilities, damages, costs and expenses (including attorney fees) arising from or related to Participant's participation. Participant agrees to indemnify and hold harmless the Organization against third-party claims arising from Participant's actions.

The parties acknowledge that certain roster and medical information is personal and confidential. The Organization will use roster and medical information solely for administration, safety, and competition purposes, and will not disclose sensitive health information except as required to obtain emergency medical care or as required by law. Governing Law
