What the Title II Request Form Is and when it applies

The Title II Request Form is a written request used primarily to ask a public entity for services, modifications, or accommodations under Title II of the Americans with Disabilities Act (42 U.S.C. §12131 et seq.). It collects claimant identity, the specific accommodation requested, circumstances or locations where the accommodation is needed, and supporting documentation. Agencies use the form to record receipt, route to an ADA coordinator, and document decisions. The form can be completed and retained electronically in compliance with the ESIGN Act (15 U.S.C. ch. 96) and state UETA rules where applicable.

Why a clear, compliant Title II Request Form matters

A structured Title II Request Form standardizes intake, documents the request and agency response, and reduces processing delays. A complete form helps establish intent, supports timely decision-making, and preserves an auditable record that meets ESIGN/UETA requirements for electronic acceptance.

Why a clear, compliant Title II Request Form matters

Who prepares or receives these requests

These roles ensure requests are documented, routed, and resolved consistently across agencies.

  • Individuals or authorized representatives submitting accommodation requests to a public entity or contractor.
  • ADA coordinators or accessibility officers who review, approve, or deny requests and assign next steps.
  • Human resources, facilities, or program managers who implement approved modifications and track completion.

Common signer and approver profiles

Requester / Advocate

  • The result was consistent compliance, faster turnaround, and centralized retention of completed forms tied to patient records for audits and care coordination.

Martin Properties

Tim Martin used electronic signing to process access and accommodation requests for properties without in-person meetings.

  • The approach reduced paper handling and scheduling friction.
  • By collecting detailed intake and attaching supporting evidence electronically, the property team improved responsiveness while tracking implementation steps and storing records securely.

Practical tips to reduce errors and speed processing

Adopt these best practices to minimize back-and-forth and to create a defensible administrative record.

Use clear, specific language
Describe the requested accommodation precisely, including where and when it is needed. Avoid generic phrases like 'reasonable help' and instead explain the functional limitation and the specific adjustment that will address it.
Attach concise supporting documents
Provide provider statements or functional assessments that directly address the limitation and need for accommodation. Redact extraneous personal information to protect privacy and speed review.
Document every interaction
Log acknowledgments, clarifying questions, received documents, and implementation steps. A complete audit trail supports consistent decisions and is valuable in case of appeals or compliance reviews.
Standardize routing and roles
Define clear internal roles for intake, medical verification, facilities scheduling, and implementation. Use conditional workflow rules to route forms to the correct coordinator automatically.
