What the Trade Request Form Is and when it’s used

The Trade Request Form is a standardized document used to instruct a financial intermediary, trading desk, or internal operations team to execute a buy, sell, transfer, or other trade on behalf of an account holder or organization. It captures essential trade details — security identifier, quantity, price instructions, settlement instructions, account and routing information, and signatory authorization — and creates a record for compliance, reconciliation, and audit. The form is used by broker-dealers, investment managers, corporate treasury teams, and authorized counterparties to ensure accurate, auditable trade execution.

Why a clear Trade Request Form matters

A precise Trade Request Form reduces execution errors, shortens processing time, and creates an auditable record for compliance with market and regulatory requirements. Clear data fields and signatures improve settlement reliability and simplify reconciliations and internal controls.

Why a clear Trade Request Form matters

Who completes and signs a Trade Request Form

Typical users range from front-office traders to back-office operations and authorized client signatories.

  • Broker-dealers and trading desks: Front-office staff prepare instructions; operations validate and process trades before submission.
  • Corporate treasury and finance teams: Treasury enters or approves trades for cash management, FX, or securities transfers.
  • Registered investment advisers and custodians: Advisors prepare client-directed transactions; custodians require authorized client signatures or delegated instructions.

Assigning the correct role to each signer and reviewer reduces authorization delays and regulatory risk.

Core elements that make a Trade Request Form effective

A professional form groups trade data, account details, authorization, and routing into clear sections to speed processing while supporting audit and compliance needs.

Trade details

Specify instrument identifier (ticker/CUSIP/ISIN), buy or sell direction, order type (market/limit), and explicit price instructions to prevent execution ambiguity.

Quantity and settlement

State quantity, unit (shares, bonds, lots), settlement date, and whether partial fills are allowed; include settlement instructions and counterparty details.

Account information

Include account number, account type, broker routing, and any broker-dealer account reference to ensure the trade posts to the correct ledger.

Authorization

Designated signer name, title, signature, and date with any delegated authority references or POA details required for execution.

Compliance checks

Fields for AML/know-your-customer flags, blackout-period confirmations, pre-trade approvals, and internal control checkboxes.

Audit metadata

Capture preparer identity, timestamps, IP or session data for e-submissions, and a reference number for downstream reconciliation.

Essential fields required on every Trade Request Form

Trade Date: MM/DD/YYYY format
Security ID: Ticker, CUSIP, or ISIN
Quantity: Numeric units specified
Price Type: Market, limit, or limit price
Account Number: Full broker/custodian account
Authorized Signer: Name and title required

Step-by-step: completing and submitting a Trade Request Form

Follow a consistent sequence: prepare the form, validate details, obtain required approvals, then submit to the executing counterparty or internal desk.

  • 01
Prepare the form: Complete all trade and account fields accurately.
  • 02
Validate details: Confirm identifiers, quantities, and settlement instructions.
  • 03
Authorize signature: Collect required signatory approval and dated signature.

Submit and confirm: Send to the desk and obtain execution or rejection notice.

Configuring a digital workflow for Trade Request Forms

Map fields, authentication, routing, and archiving before deploying electronic submissions to reduce processing exceptions.

Field Configuration
Authentication Email link, SMS code, or stronger KBA
Routing Sequential reviewer order with conditional approval
Notifications Email alerts and reminders for pending approvals
Archive Secure storage with immutable audit trail

Typical electronic submission flow

Electronic trade requests follow a short workflow that preserves evidence of intent and provides an audit trail for compliance.

  • Upload form: Attach PDF or DOCX version for signing.
  • Place fields: Add signature, date, and required text fields.
  • Send to signer: Deliver via secure email link or portal.
  • Execute and store: Capture signed copy and audit data.

Technical and file requirements for eSubmission

Confirm supported file types and integration options before launching the digital workflow.

  • File formats: PDF, DOCX, and fillable forms supported
  • Integrations: CRM, custody, and trading systems available
  • Authentication: Email, SMS, or advanced verification

Common deadlines and processing cutoffs to observe

Trade processing is time-sensitive; observe market cutoffs, settlement windows, and internal reconciliation schedules to avoid fails and penalties.

Exchange cutoffs:

Submit before exchange or broker cutoff times for same-day handling

Settlement cycle:

Most securities settle on T+2 unless otherwise specified

End-of-day processing:

Late submissions may defer to next trading day

Monthly reconciliation:

Submit supporting documentation for month-end matching

Regulatory reporting:

Meet any report filing windows imposed by regulators or exchanges

Frequent mistakes that delay or block trade processing

  • Missing or incorrect account numbers cause allocation errors and settlement failures that take hours or days to resolve.
  • Ambiguous instrument identifiers (wrong ticker or omitted CUSIP) lead to execution mistakes or canceled orders and reconciliation disputes.
  • Unsigned or undated authorizations invalidate a request and require re-approval, adding operational overhead and missed trading opportunities.
  • Late submissions after broker cutoff push execution to the next trading day and may expose counterparties to price movement risk.

Key risks and possible regulatory consequences

Trade rejection: Order canceled or delayed
Settlement failure: Fail-to-deliver or receive
Regulatory fines: Penalties for reporting noncompliance
Unauthorized trade: Dispute and restitution risk
Tax reporting issues: Incorrect basis or reporting
Operational delays: Additional manual remediation
