What a Transport Request Form Is and when it’s used

A Transport Request Form is a standardized document used to request, authorize, and record the movement of goods or passengers between specified locations. It collects requester and consignee details, pickup and delivery addresses, cargo description, weight and dimensions, required equipment, requested pickup windows, and signatures. Organizations use the form to coordinate carriers, schedule pickups, and support billing and claims. Electronic versions can be completed and retained under U.S. e-signature law and integrated into transportation management systems for faster routing and audit-ready records.

Why using a clear Transport Request Form matters

A complete form reduces routing errors, supports insurance and claims, documents authorization, and creates an auditable record. Properly completed forms speed carrier acceptance, improve on-time performance, and help control costs while supporting compliance with contractual and regulatory requirements.

Why using a clear Transport Request Form matters

Who typically prepares and signs this form

Logistical workflows require multiple roles to create and approve transport requests; the following profiles are most common.

  • Logistics coordinator — Prepares requests, confirms carrier availability, and tracks pickup and delivery status.
  • Fleet manager — Assigns vehicles and drivers, verifies equipment suitability, and signs off on special handling needs.
  • Freight broker / Carrier dispatcher — Accepts loads, confirms pickup windows, and provides carrier identification and ETA.

Forms may also be completed by third-party logistics providers or by automated systems that generate requests from orders.

Essential sections to include on a professional Transport Request Form

A professional form groups information to reduce ambiguity and speed handling: requester and consignee details, pickup/drop specifics, cargo data, handling instructions, authorization, and administrative data for billing and claims.

Cargo Description

Commodity description, count, gross/net weight, palletization, dimensions, hazardous material class if applicable, and seal numbers for secure chain-of-custody.

Special Handling

Temperature control, liftgate needs, tail-lift, white-glove service, or scheduling constraints; clearly note any timed delivery requirements or appointment references.

Authorized signer name, title, signature, and date; signature block should state the authority to bind the requesting organization for charges and liability.

Insurance & References

  • Email to Dispatcher: Send signed PDF to dispatcher email on file for the load.
  • TMS Upload: Attach form to the shipment record in your transport management system.
  • Physical Handover: Provide printed copy to driver if required at pickup.

  • Integrations: Salesforce, NetSuite, Microsoft 365, and Google Workspace supported
  • File types: PDF, DOCX accepted; signed PDF preferred
  • Authentication: Email, SMS code, or advanced signer authentication

Typical timing and processing expectations

Set clear internal deadlines so carriers have time to accept and schedule pickups; communicate expectations to all parties to avoid delays.

Request lead time:

Submit requests at least 24–48 hours before desired pickup to secure carrier capacity.

Carrier confirmation:

Expect carrier acceptance or rejection within 2–8 hours during business periods.

Pickup window adherence:

Driver should arrive during the agreed window; update ETA as required.

Claims reporting:

File cargo claims within 7 days of delivery for visible damage in most carrier contracts.

Record retrieval:

Signed forms should be available for audit within 24 hours after completion.

Common mistakes that cause delays or disputes

  • Incomplete addresses or missing delivery contact information causes re-routes, missed deliveries, and extra charges from carriers.
  • Incorrect weight or dimension entries lead to reweighs, reclassification charges, and possible refusal by the carrier for safety reasons.
  • Unsigned or improperly authorized forms produce billing disputes and can invalidate carrier acceptance under contract terms.
  • Failure to disclose special handling or hazmat status can result in regulatory violation fines and denied insurance coverage.

Risks and potential consequences of incorrect or missing data

Delayed Shipment: Rescheduling fees or missed delivery windows
Claim Denial: Insurance may deny cargo claims for incomplete forms
Financial Penalties: Carrier or contractual penalties for noncompliance
Contract Breach: Potential liability for missed obligations
Insurance Denial: Missing declarations can void coverage
Regulatory Fines: Hazmat or licensing violations may incur fines
