TRAVEL CONSENT AGREEMENT

This Travel Consent Agreement (the Agreement) is entered into effective as of by and between: Consenting Party: (hereinafter "Consenting Party") and Traveling Party: (hereinafter "Traveling Party"). 5. REPRESENTATIONS, WARRANTIES AND CUSTODY

Consenting Party further represents that there are no court orders, custody agreements, restraining orders, or other legal impediments that would prohibit the minor from traveling as authorized herein.

6. INDEMNIFICATION

To the fullest extent permitted by law, Consenting Party agrees to indemnify, defend and hold harmless Traveling Party and its agents, employees and representatives from and against any claims, liabilities, losses, damages, costs and expenses (including reasonable attorneys' fees) arising out of or related to (a) any misrepresentation by Consenting Party in this Agreement; (b) any failure to disclose material information concerning the minor's health or legal status; or (c) any claim by a third party arising from the grant of consent herein.

7. NOTICES

All notices required or permitted under this Agreement shall be in writing and delivered to the addresses set forth below by hand, certified mail, or other reliable delivery service, and shall be deemed given when received.

8. MISCELLANEOUS

Governing Law: This Agreement shall be governed by and construed in accordance with the laws of the jurisdiction in which Consenting Party resides, without regard to conflict of laws principles.

Entire Agreement: This Agreement constitutes the entire agreement between the Parties with respect to the subject matter hereof and supersedes all prior and contemporaneous agreements, whether written or oral.

Severability: If any provision of this Agreement is determined to be invalid or unenforceable, the remainder of this Agreement shall remain in full force and effect to the maximum extent permitted by law.

Amendments and Waiver: No amendment, modification, or waiver of any provision of this Agreement shall be effective unless in writing and signed by both Parties. Failure to enforce any provision shall not constitute a waiver of future enforcement.

Counterparts: This Agreement may be executed in counterparts, each of which shall be deemed an original and all of which together shall constitute one and the same instrument.
