Scope of Travel and Authorization

The Parent/Guardian hereby authorizes the Authorized Person to travel with the Minor and to take all customary actions reasonably necessary for the Minor's care, safety, and transportation as set forth below. Detailed itinerary, destinations, permitted activities, and any limitations must be described in full.

Medical Authorization

The Parent/Guardian authorizes the Authorized Person to consent to any medical, dental, or surgical care and treatment for the Minor as may be deemed necessary by licensed medical professionals in the event the Parent/Guardian cannot be reached. This authorization includes, without limitation, administration of prescription medication, emergency surgery, and diagnostic testing.

Payment Terms

Confidentiality

Any intentional disclosure of confidential information in violation of this provision shall be a breach of this Agreement.

Governing Law; Entire Agreement

This Agreement shall be governed by and construed in accordance with the laws of the jurisdiction specified below. This Agreement constitutes the entire agreement between the parties with respect to the subject matter herein and supersedes all prior negotiations, understandings, and agreements, whether written or oral.
