RECITALS

WHEREAS, Organizer Name: is in the business of arranging and providing travel, accommodation, transportation and related services for group travel (the "Organizer");

WHEREAS, Participant Name: desires to enroll in the travel package titled Trip Name: for Destination: on the terms set forth below; and

WHEREAS, the parties wish to set forth their respective rights and obligations concerning the provision of travel services, payment, cancellation and liability.

SCOPE OF TRIP SERVICES

The Organizer will provide the travel services described below and in any separately attached itinerary. The scope includes transportation arrangements, lodging reservations, guided activities and on-trip coordination. Special requests and exclusions must be listed in the field below.

PAYMENT TERMS

Participant shall pay the fees set forth below in consideration of the Organizer's performance. All amounts are stated in the currency specified in this form and are due according to the schedule below. Failure to pay by the due dates may result in cancellation and applicable cancellation fees.

Participant acknowledges optional travel insurance is recommended. If Participant elects Organizer-assisted insurance purchase, costs will be itemized and charged to Participant as part of the total package price.

This agreement commences on Trip Start Date and ends on Trip End Date, unless earlier terminated in accordance with this section.

CONFIDENTIALITY

Each party agrees that confidential information received in connection with booking or performance of travel services, including but not limited to pricing, supplier contracts and participant personal information, shall be used solely for the purposes of performing obligations under this agreement and shall not be disclosed to third parties except as required by law or with prior written consent.

LIABILITY, RELEASE, AND MEDICAL INFORMATION

Participant acknowledges that travel involves inherent risks. Participant releases Organizer, its affiliates and agents from liability for personal injury, property loss or other claims except in cases of gross negligence or willful misconduct. Participant agrees to indemnify and hold Organizer harmless from claims arising from Participant's acts or omissions.

GOVERNING LAW

This Agreement shall be governed by and construed in accordance with the laws of the State of without regard to its conflict of law principles. Venue for any dispute shall lie in the courts located within that state.

ENTIRE AGREEMENT

This document, together with any attached itinerary or written additions signed by both parties, constitutes the entire agreement between Organizer and Participant with respect to the subject matter hereof and supersedes all prior negotiations, representations or agreements, whether written or oral. Any modification must be in writing and signed by both parties.
