What the Vendor Selection Criteria Form Is

A Vendor Selection Criteria Form is a structured document used during procurement to evaluate potential suppliers against predefined standards. It collects company details, financial stability, compliance certifications, performance metrics, insurance and references, and scoring across technical, commercial, and service categories. Organizations use it to compare bidders objectively, document decision rationale, and create an audit trail supporting contract awards and vendor onboarding.

Why a Clear Criteria Form Matters

A consistent Vendor Selection Criteria Form reduces bias, speeds evaluation, and creates a documented audit trail for procurement decisions. It helps purchasers compare offers on measurable factors and supports vendor accountability during contract performance.

Why a Clear Criteria Form Matters

Who Typically Completes or Uses This Form

This form is used by teams that manage procurement, risk, and vendor relationships as part of a structured sourcing process.

  • Procurement teams responsible for sourcing and scoring suppliers against objective criteria during RFPs and RFQs.
  • Contract managers who verify contractual readiness, insurance, and SLA alignment before award and onboarding.
  • Compliance and risk officers who confirm certifications, security controls, and regulatory fit for sensitive projects.

Use the form as a shared, auditable record to support selection decisions and as an input to contract terms and onboarding checklists.

Primary Roles Involved

Procurement Officer

A procurement officer completes scoring sections, aggregates vendor responses, and documents evaluation rationale. They coordinate cross-functional review, confirm required certifications, and ensure the chosen vendor meets procurement policy and budget constraints.

Vendor Representative

A vendor representative supplies company information, references, insurance certificates, pricing, and clarifications. Accurate, timely responses reduce follow-up requests and improve scoring during the evaluation phase, shortening procurement cycle time.

Core Sections of a Professional Vendor Selection Criteria Form

A complete form groups vendor data, evaluation metrics, compliance checks, scoring rules, references, and recommended actions to make comparisons straightforward and defensible.

Vendor Identification

Company legal name, DBA, address, tax ID, primary contact, and point-of-contact information for contracting and invoicing purposes.

Financial Health

Requested financial statements, credit references, and minimum turnover or bonding requirements used to assess stability and project delivery risk.

Technical Capability

Evidence of relevant experience, staffing plans, certifications, sample deliverables, and performance metrics tied to the solicitation requirements.

Compliance and Security

Required regulatory or security attestations (HIPAA, SOC 2, ISO), insurance limits, and evidence of data protection practices for sensitive work.

Commercial Terms

Pricing structure, payment terms, warranty provisions, service levels, and any standard exclusions that affect total cost of ownership.

Evaluation Scoring

Scoring methodology, weighted criteria, pass/fail thresholds, and a recommended award rationale field to capture evaluator conclusions.

Required Fields and Short Data Checklist

Common Mistakes to Avoid When Preparing the Form

  • Using inconsistent scoring weights across evaluators, which undermines fairness and creates audit challenges during contract award reviews.
  • Failing to require current insurance certificates or expired certificates, leaving the organization exposed to uninsured vendor activity.
  • Requesting unnecessary sensitive data without a lawful basis or data protection controls, increasing privacy and compliance risk.
  • Not documenting evaluation decisions or tie-breakers, which can lead to protests or challenges in public procurement contexts.

Practical Tips for Accurate, Efficient Completion

Adopt consistent controls and templates to reduce rework and accelerate vendor evaluation.

Standardize Scoring and Weights
Use a preapproved scoring template with numeric weights and pass/fail thresholds. Train evaluators to apply criteria consistently and require written rationale for any deviations to maintain a defensible procurement record.
Require Current Documentation
Mandate current insurance certificates, active certification documents, and dated financial records. Stipulate minimum validity periods to avoid receiving expired credentials at bid time.
Use Digital Collection and Audit Trails
Collect forms electronically to capture timestamps, file versions, and signer metadata. Audit trails streamline review, support legal defensibility, and reduce administrative follow-ups for missing items.
Limit Sensitive Data Requests
Request only necessary personal data and protect it under access controls. For healthcare or financial contexts, document legal basis for processing and limit distribution to essential reviewers.
