Recitals

WHEREAS, Host Entity: operates and controls the premises and facilities identified above;

WHEREAS, Visitor desires to enter the premises for the purpose of observation, inspection, participation in a guided visit, or other authorized activity described in the Scope of Visit below; and

WHEREAS, Host requires that visitors acknowledge known and unknown risks and execute this Release and Indemnity as a condition of entry.

Acknowledgment of Risk

Visitor expressly acknowledges that entry onto the premises and participation in the activities described above involve inherent risks, including but not limited to trips, slips, falls, exposure to tools, machinery, uneven surfaces, and acts or omissions of third parties. Visitor understands that these risks may result in personal injury, illness, property damage or death.

Visitor hereby knowingly and voluntarily assumes all such risks, whether known or unknown, and accepts full responsibility for any injury, damage or loss sustained during the visit, except to the extent caused by Host's gross negligence or willful misconduct.

Release and Waiver

In consideration of being permitted to enter the premises, Visitor, on behalf of Visitor and Visitor's heirs, assigns, personal representatives and next of kin, releases, waives, discharges and covenants not to sue Host Entity: , its officers, directors, employees, agents, affiliates and contractors (collectively, "Released Parties") from any and all liability for any loss, damage, injury or death, and any claim or demand therefor, whether arising in tort, contract or otherwise, including claims arising out of the ordinary negligence of the Released Parties, to the fullest extent permitted by law.

Indemnification

Visitor agrees to indemnify, defend and hold harmless the Released Parties from and against any and all claims, demands, causes of action, losses, liabilities, fines, penalties, costs and expenses (including reasonable attorneys' fees) arising out of or related to Visitor's presence on the premises, any breach of this agreement by Visitor, or Visitor's negligent or intentional acts or omissions.

Medical Authorization and Emergency Contact

In the event of an emergency, Visitor authorizes Host and its agents to obtain emergency medical treatment for Visitor if Visitor is unable to consent. Visitor acknowledges that Visitor is responsible for all costs of such treatment and agrees to indemnify Released Parties for arranging or facilitating emergency care.

Minor Visitor

  Visitor is under 18 years of age. If checked, the parent or legal guardian must complete the Guardian section below and sign on behalf of the minor.

Photography and Media Release

Visitor consents to the photographing, videotaping and recording of Visitor by or on behalf of Host during the visit and grants Host the perpetual, royalty-free right to use such images and recordings for promotional, educational or other lawful purposes. Visitor may opt out by notifying Host in writing prior to entry.

Governing Law and Venue

This Agreement shall be governed by and construed in accordance with the laws of the State of . Venue for any dispute shall lie exclusively in state or federal courts located within that state.

Acknowledgment and Certification

Visitor certifies that Visitor has read this document in its entirety, understands its terms, and voluntarily agrees to its provisions. Visitor acknowledges that this release is intended to be as broad and inclusive as permitted by applicable law, and that if any portion is held invalid, the remainder shall continue in full force and effect.

Entire Agreement

This document constitutes the entire agreement between Visitor and Host with respect to the subject matter hereof and supersedes all prior or contemporaneous oral or written agreements, understandings and representations. No modification shall be effective unless made in writing and signed by both parties.
