What the WC EFT Payment Authorization Form Is

The WC EFT Payment Authorization Form authorizes a payer (insurance carrier, state board, or employer) to deposit workers' compensation payments directly into a designated bank account via electronic funds transfer (EFT). The form collects payee identity, routing and account numbers, bank name, account type, and consent to recurring deposits and adjustments. Properly completed, it speeds distribution, reduces paper checks, and provides an auditable consent record suitable for electronic storage. The record typically includes signature, date, and notice of how to revoke authorization. It is used across insurers and self-insured employers.

Why employers, carriers, and payees rely on this form

The WC EFT Payment Authorization Form reduces payment delays, lowers check-handling costs, and creates a clear electronic consent trail for audits and compliance. It standardizes payee banking data, minimizes returned-payment risk, and simplifies recurring disbursements for ongoing claims.

Why employers, carriers, and payees rely on this form

  • Employers: payroll or benefits contacts provide payee bank details for claim payments.
  • Insurers/TPAs: use the form to set up recurring disbursements and audit trails.
  • Payees: injured workers or providers authorize direct deposit and confirm tax withholding status.
