By signing this form the Payer authorizes the Company named above to debit the specified account on a weekly basis for the Weekly Debit Amount. This authorization permits the Company to initiate debit entries and, if necessary, credit entries to correct any erroneous debit. Debits will continue until the authorization is revoked in writing as provided below.

Payer agrees to provide written notice of cancellation or change at least five (5) business days prior to the next scheduled debit. The Payer understands that cancellation or a change request received less than five (5) business days prior to a scheduled debit may not be effective for the next scheduled transfer. Payer further agrees to promptly notify the Company of any change to bank account or routing information.

In the event an ACH debit is returned for insufficient funds or other reason, the Payer remains obligated for the amount of the debit plus any returned item or processing fees assessed by the banking institution. The Company reserves the right to suspend ACH privileges and re-submit returned debits. The Payer releases and indemnifies the Company from any claim arising from debits made in accordance with this authorization, except for debits made in error by the Company. This authorization is governed by applicable federal and state law.
