BRIEF DESCRIPTION OF ACCIDENT Employees were pumping concrete into bridge beam forms for a pedestrian walkway. Approximately 130 feet of a 392 foot pour had been completed when a failure occurred. Six employees fell approximately 16 feet onto a sandy river bank. The form work for the bridge beam forms collapsed during the pour. Three employees were treated and released, two employees were hospitalized over night for observation, and the sixth employee was hospitalized with three crushed vertebrae (two in the lumbar and one in the cervical spine). ACCIDENT PREVENTION RECOMMENDATIONS 1. Construct form work in accordance with engineering recommendations. 2. Maintain a copy of bridge work blue prints on the jobsite for reference. Note: This case described was selected as being representative of improper work practices which likely contributed to a fatality from a fall and/or electrocution.
