What a Well Completion Document Is

A Well Completion Document is a formal record created after a well (such as an oil, gas, or water well) reaches the end of its drilling and completion phase. It compiles technical details about casing, cementing, perforations, downhole equipment, production tests, and final well configuration, plus inspection and regulatory submissions. Operators, service companies, and regulators use it to certify the well's status, safety measures, environmental controls, and production capacity. The document supports regulatory compliance, subsequent operations, and long-term recordkeeping for asset management and liability purposes.

Why the Well Completion Document Matters

Using a Well Completion Document centralizes technical, safety, and regulatory data in a single authoritative record. It reduces inspection delays, simplifies permit closeout, supports environmental reporting, and provides evidence for warranty, insurance, and ownership claims while enabling consistent handoff between field teams and central operations.

Why the Well Completion Document Matters

Who Prepares and Relies on This Record

Primary users include operators, completion engineers, regulators, and field service contractors who prepare and approve completion records.

  • Operators and asset managers responsible for certification, maintenance planning, and regulatory submission.
  • Completion engineers and service companies documenting downhole operations, tests, and equipment specs.
  • State oil and gas regulators reviewing compliance, environmental controls, and production reporting.

Smaller stakeholders such as insurers, landowners, and legal counsel rely on completion records for claims, leases, and title verification.

Step-by-Step: Completing the Document

Follow this stepwise procedure to complete, review, and submit a Well Completion Document to stakeholders and regulatory agencies.

  • 01
Prepare Data: Assemble logs, casing and cement records, test results, and equipment serial numbers.
  • 02
Draft Report: Populate fields with measured values, well overview, and completion narrative.
  • 03
Internal Review: Obtain technical sign‑off and safety validation from engineering and operations teams.
  • 04
Submit & File: Send to regulator, retain originals, and distribute copies to stakeholders.

Typical Digital and Paper Workflow

This workflow shows common digital and paper steps for preparing, signing, and distributing a Well Completion Document.

  • Upload: Upload final PDF and supporting attachments to the document management system.
  • Tag Fields: Place signature, date, and required data fields for each signer role.
  • Authenticate: Choose authentication: email link, SMS code, or organization SSO for signers.
  • Archive: Store signed records with audit trail, attach to asset file, and backup.

Configuring an Online Approval Workflow

Configure your online workflow to match approvals, signer order, and required authentication for Well Completion Document processing.

Field Configuration
Signing Order Sequential: Operator, Completion Engineer, Regulator required for filing
Auth Method Email link with optional SMS code or SSO for higher assurance
Required Attachments Cement reports, pressure tests, equipment serial numbers, and lab certificates
Retention Policy Maintain signed record in central repository for minimum seven years

Platform and Integration Considerations

Typical platform requirements for digital completion, signing, and secure storage of Well Completion Documents in enterprise environments.

  • Formats: PDF, DOCX, XLSX support
  • Integrations: CRM, ERP, cloud storage systems
  • Authentication: Email, SMS, SSO, or KBA

Core Sections to Include in the Document

Essential parts of a professional Well Completion Document include technical summaries, compliance sections, and attachments that substantiate completion performance and safety.

Executive Summary

Provide a concise overview of the well objectives, final operational status, key performance measurements, any notable incidents during completion, and an explicit statement that work meets specified acceptance criteria.

Wellbore Details

Document casing strings, centralizer placement, packer settings, perforation intervals, and measured depths. Include as-built diagrams, referenced logs, and downhole telemetry summaries to support technical assertions.

Testing & Results

Summarize pressure, flow, and integrity tests with measured values, units, tolerances, and interpreted conclusions. Attach raw test datasets and technician certifications to verify results.

Equipment Inventory

List downhole and surface equipment by model, serial number, inspection status, and warranty information. Specify items intentionally left in the well and items recovered during completion.

Environmental Controls

Detail containment measures, produced water handling, emissions controls, spill prevention practices, and monitoring results. Reference permits, environmental thresholds, and mitigation steps implemented during completion.

Signatures

Include dated signature blocks for responsible parties and engineer certifications. Add notarization or witness sections when legally required. Record signer names, titles, organization, and contact details for future inquiries.

Common Attachments and Downloadable Files

Common downloadable items attached to a Well Completion Document include formatted PDF reports, raw test datasets, and certified lab attachments for recordkeeping.

Final Report (PDF)

A locked, searchable PDF combining narrative, tables, diagrams, and embedded signed certificates suitable for regulatory submission and archival storage. Consider PDF/A for long-term preservation.

Raw Data Files

CSV or Excel files containing time-stamped test logs, pressure and flow readings, and instrument calibration records. Provide metadata for traceability.

Lab Certificates

Scanned or native lab reports showing analytical results, chain-of-custody, and technician signatures. Attach originals to support compliance audits.

Maps & Diagrams

Geolocation maps, sectional well diagrams, and cross-sections in standard image or CAD formats, labeled with coordinates and datum for regulator review.

Security, Compliance, and Technical Controls

Encryption: TLS 1.2/1.3 in transit; AES-256 at rest
Certifications: SOC 2 Type II; ISO 27001; PCI DSS
HIPAA: BAA available for protected health information
Audit Trail: Timestamps, IPs, and signer actions recorded
Standards: ESIGN and UETA legal compliance supported
Accessibility: WCAG 2.0 Level AA compliant

Consequences of Incomplete or Incorrect Records

Regulatory Fines: Fines, stop-work orders, or permit suspension
Environmental Liability: Cleanup costs and civil penalties
Operational Delays: Delayed production and revenue loss
Insurance Claims: Denial of coverage for defects
Contractual Breach: Indemnity claims and litigation risk
Record Rejection: Incomplete filings may be rejected

Common Preparation Pitfalls to Avoid

  • Incomplete measurements or missing test reports cause regulator rejections and require time-consuming resubmissions, increasing project delays and cost overruns.
  • Inconsistent naming or incorrect API numbers prevent matching in state databases, leading to permit status ambiguity and potential enforcement actions.
  • Failure to capture signed certifications from responsible engineers can invalidate technical claims and expose operators to liability and insurance disputes.
  • Storing only paper originals without digital backups risks loss, slower retrieval, and difficulty meeting rapid information requests during audits or incidents.

Practical Examples of Use

Real-world examples show how Well Completion Documents are used to resolve technical, regulatory, and commercial issues.

Operator Example

An operator filed a complete report after a complex recompletion to document new perforations and production tests.

  • The regulator accepted online submission promptly.
  • The digital record shortened review time, avoided multiple site visits, and supported fast payment release to contractors while preserving audit-ready evidence for future inspections.

Service Company Example

A service company provided instrument-calibrated test logs and lab certificates with its completion package.

  • The operator required certified attachments for acceptance.
  • Including standardized raw data and chain-of-custody forms reduced follow-up questions, accelerated acceptance, and improved the traceability of test results for compliance audits.
