What a Work Comp Request Is and when it's used

A Work Comp Request is a formal document used to notify an employer and insurer that an employee suffered a work-related injury or occupational illness and is requesting workers' compensation benefits. The request typically records claimant identity, injury date, location, a description of the incident, treating providers, and any lost-time or wage information. Employers, insurers, and state workers' compensation agencies use the request to open a claim, initiate medical care approvals, and determine benefit eligibility. Correctly completed requests help speed triage, medical authorization, and benefit decisions.

Why a clear Work Comp Request matters

A complete, accurate Work Comp Request reduces delays, limits disputes about causation or eligibility, and preserves claim rights. It creates an auditable record for the employer, insurer, medical providers, and state agency while supporting timely treatment and wage-loss calculations.

Why a clear Work Comp Request matters

Who fills out and relies on the Work Comp Request

Different parties prepare or act on Work Comp Requests; clarity about roles reduces processing friction and ensures the claim advances promptly.

  • Injured employees or their representatives who must report the injury and provide accurate incident details for a claim.
  • Supervisors or HR representatives who document initial incident reports, witness statements, and safety observations.
  • Insurer claims handlers who use the request to open a claim, approve initial medical care, and assign an adjuster.

Each party should complete only the sections relevant to them and keep a copy of the submitted request for the claim file and regulatory recordkeeping.

Essential sections every professional Work Comp Request includes

A structured request reduces follow-up and supports accurate benefit decisions; include these six components consistently.

Claimant

Full legal name, date of birth, job title, employee ID, and best contact details so the claim is reliably attributed and communications reach the injured worker.

Injury Date

Exact date and time of the incident using MM/DD/YYYY and 24-hour or AM/PM notation; precise timing affects benefit eligibility and indemnity calculations.

Incident Location

Street address and specific work area or job site description; location determines jurisdiction and which employer unit or policy applies.

Incident Description

Clear factual narrative of what happened, sequence of events, tools or equipment involved, and witness names to support causation determinations.

Medical Info

Name and contact of treating provider(s), initial diagnosis, and whether first aid or referral occurred; essential for authorization and continuity of care.

Work Status

Whether the claimant returned to work, is on restricted duty, or off work, plus expected return dates—used for wage-loss and light-duty planning.

Stepwise completion checklist for a Work Comp Request

Follow these four practical steps to prepare and submit a complete request.

  • 01
Gather facts: Collect claimant info, witnesses, and provider contacts.
  • 02
Document incident: Write a clear, factual description without opinion.
  • 03
Attach records: Include initial medical reports and time-off notes.
  • 04
Submit and retain: Send to employer, insurer, and file a copy locally.

Configuring an online Work Comp Request workflow

Set up fields and routing to automate claim intake and reduce manual handoffs.

Field Configuration
Claimant Info Required text fields with ID validation
Incident Details Long-text input with conditional fields
Medical Attachments File upload allowed (PDF, DOCX)
Routing Rules Auto-send to claims team and HR

Where to send a completed Work Comp Request

Determine intended recipients before submission so routing and acknowledgements occur without delay.

  • Employer HR: Primary recipient for internal incident investigation and payroll tracking.
  • Workers' Insurer: Claims unit opens the claim and authorizes initial care.
  • Treating Provider: Provider receives authorization and claim reference details.
  • State Agency: Submit only if state rules require agency filing or notice.

Digital submission and technical compatibility

Confirm accepted formats and integrations before uploading documents to avoid re-submission delays.

  • File Formats: PDF, DOCX, JPEG supported
  • Authentication: Email, SMS code, or multi-factor
  • Integrations: HR and claims systems

Use secure connections (TLS) and platform audit trails to preserve evidence of submission, signer identity, and time stamps.

Typical timing and reporting expectations

Timing rules vary by employer policy and state law; report and document incidents promptly to preserve claim rights.

Immediate employer notice:

Notify employer as soon as practicable; many employers require same-day or next-business-day notice.

Insurer claim submission:

Insurers generally request initial reports within 24–72 hours of employer notification.

State filing window:

Filing deadlines for formal claims vary by state; check your state workers' compensation board for exact windows.

Medical authorization:

Seek initial treatment immediately; authorization and network rules affect billing and continuation of care.

Appeal deadlines:

Appeal timeframes vary by jurisdiction; a prompt claims file preserves appeal rights.

Key milestones from incident to claim decision

A typical claim advances through these stages; tracking each milestone clarifies responsibilities and expected timing.

01

Incident Reported

Employee notifies employer and incident is documented.

02

Claim Opened

Employer or insurer opens a claim file and assigns an adjuster.

03

Medical Evaluation

Provider documents diagnosis and work status.

04

Decision Issued

Insurer issues acceptance, denial, or request for additional info.

Common mistakes to avoid when preparing a Work Comp Request

  • Incomplete claimant data that prevents identity verification and delays benefit payments or medical authorizations.
  • Vague or opinion-based incident descriptions that leave causation and compensability unclear to adjusters.
  • Missing medical provider information or failing to attach initial reports that are needed for treatment approval.
  • Failure to follow employer reporting protocols or notify the insurer in the timeframe required by policy.

Potential consequences of incorrect or late requests

Late Filing: Claim denial risk
Incomplete Data: Delayed benefits
Misrouted Claim: Coverage dispute
Incorrect Medical Codes: Billing rejections
Privacy Breach: HIPAA violation risk
Employer Penalty: Possible regulatory fines

Attach medical documentation
Include first-aid reports, provider notes, and any diagnostic results to speed medical authorization and reduce insurer requests for additional records.
Preserve originals
Keep signed original documents and create secure electronic copies with audit trails for compliance and potential litigation support.
