WHEREAS

WHEREAS, Organization Name operates youth sports programs and provides coaching, supervision, training, and related activities (the Programs) for minors; and

WHEREAS, Parent/Guardian is the lawful parent or guardian of the Participant and desires that the Participant participate in the Programs under the terms and conditions set forth in this Agreement; and

NOW, THEREFORE, in consideration of the mutual covenants herein and other good and valuable consideration, the parties agree as follows.

Organization reserves the right to suspend or remove a Participant for safety, misconduct, or material breach of rules without refund when necessary for the protection of other participants or personnel.

CONFIDENTIALITY

Each party acknowledges that personal information exchanged under this Agreement, including medical and emergency contact information, is confidential. Organization shall use personal information only for administration of the Programs, safety, and communications, and shall not disclose such information except as required by law or with Parent/Guardian's written consent. Organization will implement reasonable safeguards to protect personal information.

ASSUMPTION OF RISK, WAIVER AND MEDICAL AUTHORIZATION

Parent/Guardian acknowledges that participation in sports involves risk of injury, including serious bodily injury or death. Parent/Guardian voluntarily assumes all risks on behalf of the Participant and releases Organization, its officers, agents, coaches and volunteers from liability for ordinary negligence to the fullest extent permitted by law. This release does not apply to willful misconduct or gross negligence.

In the event of injury or medical emergency, Organization is authorized to obtain emergency medical treatment for the Participant and Parent/Guardian agrees to be responsible for any costs incurred.

PHOTO AND MEDIA RELEASE

Parent/Guardian hereby grants Organization the right to photograph and record the Participant during events and to use such photographs, video, and audio recordings for promotional, educational, and informational purposes in any medium, without compensation, unless Parent/Guardian marks decline below.

CODE OF CONDUCT

GOVERNING LAW; ENTIRE AGREEMENT

This Agreement shall be governed by and construed in accordance with the laws of the State of , without regard to conflict of laws principles. This Agreement, together with any written program rules incorporated by reference, constitutes the entire agreement between the parties with respect to the Participant's enrollment and supersedes all prior oral or written agreements.

If any provision of this Agreement is held invalid or unenforceable, the remaining provisions shall remain in full force and effect. Any amendment to this Agreement must be in writing and signed by both parties.
