Medical Authorization & Liability

If unable to reach authorized contacts, Parent/Guardian hereby authorizes program staff to obtain emergency medical treatment for the student and to provide or consent to medical care as deemed necessary. Parent/Guardian agrees to be responsible for costs of medical treatment and to indemnify the program for costs incurred in obtaining emergency care.

Media Release

The program may from time to time take photographs or video of program activities for educational, promotional, or documentation purposes. Parent/Guardian grants the program the right to use images of the student for such purposes unless the box below is checked to indicate refusal.

Confidentiality, Records & Legal Terms

Parent/Guardian authorizes the program to maintain necessary records for the purpose of providing services under this Contract. The program will make reasonable efforts to maintain confidentiality of student records in accordance with applicable law. Parent/Guardian agrees to indemnify and hold harmless the program, its employees, volunteers and agents from claims arising out of the student's actions except where arising from the program's gross negligence or willful misconduct.

This Contract constitutes the entire agreement between the parties with respect to the subject matter herein and supersedes prior oral or written agreements.
