APPRENTICESHIP AGREEMENT

Employer will maintain records of hours, competencies, related instruction attendance and evaluations. Apprentice may request copies of records. Progress evaluations will be provided at least every six months or as otherwise required by the training program.

Health, Safety and Insurance

Employer shall provide a safe workplace and notify Apprentice of known hazards. Apprentice must comply with all safety instructions and use required protective equipment. Employer will maintain workers' compensation coverage as required by law.

Termination and Suspension

This Agreement may be terminated by Employer or Apprentice for cause, including but not limited to material breach, failure to meet safety requirements, or loss of required certifications. Termination procedures will follow applicable program policies and any legal requirements for notice and documentation.

Acknowledgments, Background Checks and Policies

By signing below, the Apprentice acknowledges receipt of program rules, policies regarding attendance, conduct and safety, and consents to any background checks or drug testing that are a condition of enrollment when required by Employer or program standards.

Minor Status and Parent / Guardian

If Apprentice is a minor, a parent or legal guardian must sign this Agreement. Indicate minor status below and provide guardian contact information.

Confidentiality and Intellectual Property

Apprentice agrees to maintain confidentiality of Employer trade secrets and proprietary information encountered during the apprenticeship. Ownership of work product and inventions created in the scope of employment will be determined in accordance with Employer policies and applicable law.

Dispute Resolution

The parties agree to attempt resolution of disputes arising under this Agreement through good faith negotiation. If unresolved, disputes shall be resolved by binding arbitration or other mutually agreed process, subject to applicable law and program requirements.
