The proponent agency is the Office of the Deputy Chief of Staff, G-1 (Director of Military Personnel Management (DMPM)).

Authority: Title 10, United States Code, Sections 3013 and 4334; Executive Order 9397.

Principal Purpose(s): To specify and record the contractual agreements and obligations for participation in the Army’s Career Satisfaction Program (CSP) – Post for Active Duty Service Obligation.

Routine Use(s): This form will be maintained in the Cadet’s military personnel records and becomes a permanent part of the subject individual’s official personnel records as confirmation of contracting, obligation, and agreements. All uses of this form are internal to the U.S. Army.

Disclosure: Disclosure of the SSN and other personnel information is voluntary. However, failure to provide the required information may result in denial of participation in the Army’s Post for ADSO Program.

Agreement of Cadet

Under the terms of the Post for ADSO Program, I request (first preference), or (second preference), as my post of choice. In exchange for receiving either post of choice as stated above I hereby agree to incur an additional three years to my commissioning ADSO depending on my ROTC scholarship level. For scholarships or non-scholarships incurring a three (3) year commissioning ADSO, my new Active Federal Commissioned Service ADSO will be six (6) years. For scholarships incurring a four (4) year commissioning ADSO, my new Active Federal Commissioned Service ADSO will be seven (7) years.

Agreement of the Department of the Army

In consideration of the agreement by the Cadet above, the Department of the Army agrees to assign me to one of the posts as stated above for my first duty station.

Acknowledgement/Statement of Understanding

1. I understand that the ADSO incurred pursuant to the Army’s Post for ADSO Program as stated above will be in addition to the initial 3 or 4 year ROTC ADSO I incurred pursuant to AR 350-100 and the terms of my ROTC contract (CC Pam 145-4, DA Form 597-3). I further understand that the ADSO incurred pursuant to the Army’s Post for ADSO Program as stated above is in addition to, and must be served consecutively with, any other ADSO I may have incurred pursuant to the Army’s Branch for ADSO Program.

2. I understand that the ADSO incurred pursuant to the Army’s Post for ADSO Program will be served consecutively and not concurrently with my initial 3 or 4 year ROTC ADSO and will begin on the day after my initial 3 or 4 year ADSO is completed. Thereafter, the ADSO incurred pursuant to the Army Post for ADSO Program will be served concurrently with any other non-statutory ADSO, unless otherwise provided by law or contract.

3. If I am an ROTC scholarship Cadet, I understand that upon completion of my initial ROTC ADSO, I will have satisfied the terms of my Cadet service agreement and that I will not be subject to reimbursement for a pro-rata share of the cost of my ROTC scholarship if I fail for any reason to serve any part of the ADSO incurred pursuant to the Army’s Post for ADSO Program.

4. I understand that if my branch is Army Nurse Corps (ANC), I must pass the National Council on Licensure Examination for Registered Nurses (NCLEX-RN) on the first attempt. Otherwise, my Post for Active Duty Service Obligation (ADSO) addendum becomes void.

5. I understand that this agreement does not supersede or replace my Cadet Service agreement and that I continue to be bound by all of the terms and conditions of my Cadet Service agreement.

6. I understand that this agreement is contingent upon my satisfying all of the necessary requirements for graduation and commissioning.

7. I understand that this agreement may be canceled by the Secretary of the Army, or his designee, at any time based on the needs of the Army.

8. I understand that the availability of my post is subject to change based on the requirements of the Army.

9. In the event that the post is not available based on circumstances not under my control or due to lack of availability to the Army, this contract will become void.

10. In the event that the Army requires me to PCS subsequent to signing into my post of choice pursuant to the Post for Active Duty Service Obligation (ADSO) Program or transfers me prior to the requirements of a normal tour of duty length as prescribed in AR 614-100 against my desire, this contract will become void.

11. In the event that I, subsequent to signing the contract, request assignment to a different post for my first assignment, and the Army consents, the ADSO I incurred from the Post for Active Duty Service Obligation (ADSO) Program remains in effect.

12. I have read and understand the statements above and understand these statements are intended to constitute all the promises and guarantees concerning my participation in the Army’s Post for ADSO Program. No other (verbal or otherwise) promise or representation not contained in this agreement is valid or will be honored. I hereby state that I have not been promised anything other than what is written on this agreement and hereby waive any claim based upon any promise or representation not contained in this agreement.
