Attendance & Conduct: I acknowledge that regular attendance and active participation are required. The program may dismiss a participant for repeated unexcused absences or conduct detrimental to the safety or learning environment. I understand that no refund will be issued for dismissals for cause unless required by applicable law.

Health & Emergency Treatment Authorization: In the event of a medical emergency, I authorize program staff to seek emergency medical treatment for the participant and to disclose necessary medical information to treating providers. I will be responsible for all costs associated with such treatment to the extent permitted by law.
