[ ] (name of assignor), on behalf of (the judgment debtor), has executed a note in my name confirming, by acknowledgment, that he is satisfied with the payment by me of a certain judgment, and that he has no objection to collection or receipt of the same, and will permit all proceedings authorized by this agreement in any court in the place where it may be in his power to institute proceedings. A copy of the writing by assignor in the case, together with a certified copy of the agreement for the payment of the money, may be served on the judgment debtor in the same manner and form as if it had been personally served upon the judgment debtor, and I authorize my agent to charge collection of the judgment with regard to said notice and execution. (2) the application for a transfer or substitution of judgment; (3) the temporary or deferred operation of the judgment; and (4) the granting of other similar relief if the court finds that the amount of the judgment is too large to be satisfied by the debtor within the period limited by law.
