Image and Likeness License: Athlete grants the Program the non-exclusive, royalty-free right to use the Athlete's name, photograph, video, voice, and biographical material for promotional, educational, and athletic purposes in any medium. Athlete understands that this license is worldwide and perpetual for materials created during the period of enrollment unless the Athlete affirmatively withholds consent below.


Medical Treatment Authorization: In the event of injury or medical emergency during participation, the undersigned hereby authorizes Program personnel to secure emergency medical care and, if necessary, transport the Athlete to a medical facility. The undersigned acknowledges responsibility for all costs not covered by insurance.

Assumption of Risk and Release of Liability: Participation in athletic activities carries inherent risks. The undersigned voluntarily assumes all risks associated with participation and releases the Program, its officers, coaches, employees, and agents from liability for injury, loss, or damage, except for willful misconduct or gross negligence.
