Assumption of Risk and Release: Participation in athletic training, strength and conditioning, sport‑specific drills, and related activities involves physical exertion and risk of injury. In consideration of the student's participation, the signer acknowledges that the organization, its coaches, employees, volunteers, and agents are not responsible for injuries sustained except to the extent caused by gross negligence. The signer voluntarily assumes all risks, foreseeable and unforeseeable, arising from participation, and hereby releases, indemnifies, and holds harmless the organization against claims, damages, or liabilities arising from such participation, except for claims resulting from willful misconduct.

Emergency Medical Authorization: If the student requires emergency medical treatment and the parent/guardian cannot be reached, the signer authorizes designated program staff to obtain medical care and to consent to any necessary treatment, including hospitalization and anesthesia, as deemed necessary by licensed medical personnel. The signer agrees to be financially responsible for such care.

Photo and Media Release: I grant permission for photographs, video, and audio recordings of the student taken during program activities to be used by the organization for educational, promotional, or instructional purposes. I understand no compensation will be provided for such use.

Attendance, Conduct & Safety Rules: The student agrees to follow all program rules, coach instructions, and facility safety policies. The organization may suspend or remove a participant for violations of conduct policies, unsafe behavior, or non‑payment of fees. Removal for disciplinary reasons does not entitle the family to a full refund; prorated refunds are at the sole discretion of program administration and per the stated refund policy below.

Cancellation and Refund Policy: Requests for withdrawal must be submitted in writing. Refunds for withdrawal prior to program start will be issued minus any administrative or processing fees. Once a program has commenced, refunds are limited and may be prorated based on sessions attended and non‑refundable fees. The organization reserves the right to cancel or reschedule sessions; in such cases, prorated refunds or credit toward future programs will be provided at the organization's discretion.

Privacy and Use of Information: The organization collects personal and medical information necessary for safe participation and program administration. Information will be maintained confidentially and disclosed only to coaches, medical personnel, and program administrators as needed for safety and program operation. The signer consents to the collection and limited use of the information provided for these purposes.
