The undersigned voluntarily assumes all such risks and agrees that the athletics program, its governing body, members, coaches, agents, and volunteers shall not be liable for injury or loss arising from participation except to the extent caused by their gross negligence or willful misconduct.

The undersigned further agrees to indemnify, defend and hold harmless the athletics program, its governing body, members, coaches, agents, and volunteers from any claims, demands, liabilities, damages, or causes of action brought by or on behalf of the student arising out of participation, except where such claims arise from gross negligence or willful misconduct of the indemnified parties.

I grant permission for the athletics program to record, photograph, or otherwise use images and video of the student for promotional, training, or archival purposes. I understand that no compensation will be provided for such use.

Termination and Refund Policy

Enrollment is confirmed upon acceptance and receipt of the required deposit. Withdrawal prior to the first official practice results in a refund of fees paid less an administrative processing fee of $. After the program has commenced, refunds are prorated at the discretion of the program and may be denied where participation has begun or where dismissal has occurred for cause. All refund requests must be submitted in writing.
