I, , on behalf of myself or on behalf of the participant named above (as the parent or legal guardian), acknowledge that participation in camp activities involves inherent risks, including but not limited to physical injury, illness, allergic reaction, property loss, and death. I freely accept and assume all risks associated with participation.

In consideration for the participant being permitted to attend and participate in the camp, I hereby release, waive, discharge and covenant not to sue the camp, its organizers, directors, officers, employees, volunteers, contractors and agents (collectively, the Released Parties) from any and all liability, claims, demands, actions or causes of action arising out of or related to any loss, damage or injury, including death, that may be sustained by the participant, whether caused by the negligence of the Released Parties or otherwise, to the fullest extent permitted by law.

I further agree to indemnify, defend and hold harmless the Released Parties from and against any and all claims, demands, losses, liabilities or expenses (including reasonable attorney fees) arising out of or related to my or the participant's acts or omissions while participating in camp activities.

Emergency Medical Treatment Authorization

If the participant requires emergency medical treatment while under camp supervision and I cannot be reached, I authorize camp staff to seek medical assistance and to consent to any emergency examination, anesthetic, medical or surgical diagnosis or treatment and hospital care deemed advisable by the attending medical professional. I understand that reasonable efforts will be made to contact the emergency contacts listed above prior to non-emergency treatment.
