Policies, Waiver, and Releases

Participation in cheerleading involves risk of physical injury, including serious bodily injury or death. The undersigned affirms that the student is physically capable of participation and accepts all risks inherent in training, practice, travel, and competition. The undersigned releases, waives, and discharges the organization, its directors, coaches, volunteers, and affiliates from liability for ordinary negligence and for injuries arising from participation, and agrees to indemnify and hold harmless for claims arising out of participation except where caused by willful misconduct.

Medical Release: The undersigned authorizes emergency medical treatment deemed necessary by licensed medical personnel. The undersigned accepts financial responsibility for treatment costs and agrees the organization may provide emergency transportation if necessary.
