By signing this form the signatory accepts the following terms and conditions and acknowledges that failure to comply may result in dismissal from the program and forfeiture of certain fees as described below.

Tuition and Fees: Tuition is due in accordance with the selected payment plan. Chemistry Foundation reserves the right to suspend participation for nonpayment. Refunds, if any, are issued in accordance with the Foundation's refund policy: refunds for withdrawal requested prior to the first scheduled session will be issued minus a nonrefundable processing fee; after a program begins, refunds are prorated at the sole discretion of the Foundation for documented extenuating circumstances.

Code of Conduct: Participants must adhere to all safety protocols and the Code of Conduct. Any conduct that endangers participants or staff, or breaches laboratory safety rules, may result in immediate removal without refund.

Intellectual Property: Materials created by participants during Foundation activities may be used by the Foundation for educational and promotional purposes. The Foundation will attribute authorship when practicable. The participant grants a nonexclusive, royalty-free license to the Foundation to use photographs, videos, and work products for Foundation purposes.

Privacy & Records: The Foundation will maintain participant records for administrative purposes and will handle personal data in accordance with applicable privacy obligations. Enrollees consent to the Foundation contacting the emergency contact and parent/guardian as necessary for health, safety, or administrative reasons.
