Policies, Authorizations & Acknowledgements

Enrollment Commitment: By signing below, Parent/Guardian commits to payment of all tuition, fees, and charges as set forth above. Tuition obligations continue through the last day of enrollment or until properly withdrawn in accordance with the withdrawal notice requirement. Failure to pay may result in suspension or termination of enrollment and collection action.

Refunds & Withdrawal: Deposit is non-refundable except as required by written agreement. Refunds for prepaid tuition are at the sole discretion of the Provider and will be processed only after written withdrawal is received and any required notice period has been satisfied.

Health, Safety & Medication: Provider will attempt to notify Parent/Guardian in non-life-threatening medical situations. Parent/Guardian authorizes Provider to administer or permit administration of medication to the child only as expressly described in the medications_details field and only upon receipt of a signed medication administration authorization. Parent/Guardian must supply all prescribed medication in its original labeled container.

Emergency Medical Treatment: If immediate medical treatment is required and Parent/Guardian cannot be reached, Parent/Guardian authorizes Provider to secure emergency medical care for the child and to transport the child to a medical facility. Provider will make reasonable efforts to notify Parent/Guardian and emergency contacts as soon as practicable.

Photo/Video Release: I consent to photographic and video recordings of my child for educational, promotional, and internal use by Provider. I understand the recordings may be used without further notification or compensation unless I withdraw consent in writing.

Confidentiality & Records: Provider will maintain records in accordance with its recordkeeping policies. Provider will disclose records to third parties only as required by law or with Parent/Guardian written consent, except as necessary for emergency medical care or public health reporting.

Termination: Provider may suspend or terminate services for nonpayment, failure to comply with health or safety policies, or for conduct that endangers the welfare of child or others. Provider will provide written notice of termination where practicable.

Liability & Indemnification: Parent/Guardian releases Provider and its employees from liability for incidents arising from ordinary risks associated with childcare, except for willful misconduct or gross negligence. Parent/Guardian agrees to indemnify and hold Provider harmless from claims arising from Parent/Guardian's willful acts or omissions.

Acknowledgment: Parent/Guardian acknowledges receipt of Provider's policies and handbook, understands the terms of enrollment, and agrees to comply with all rules, policies, and payment obligations set forth herein.
