Registration is subject to payment of tuition and any required deposit. Participant acknowledges that failure to remit payments by stated due dates may result in suspension of access to course materials, ineligibility to receive CME credit, and collection action for unpaid balances. Refunds, if any, are governed by the Provider's refund policy below.

Completion Requirements: Participant must satisfy attendance and assessment requirements as specified by the Provider to receive CME credit. Credits will be awarded only upon successful completion and any applicable assessment submission.

Accreditation and Certificates

Provider represents that the activity is offered in accordance with applicable accreditation standards and that Participant will be issued an official certificate documenting earned CME credits upon satisfactory completion. Certificates remain the property of Provider until all financial obligations are satisfied.

Privacy, Recording and Intellectual Property

Participant authorizes Provider to collect and use personal information necessary for administration of the activity. Provider may record sessions for educational purposes; Participant consents to use of recorded materials by Provider for non-commercial educational purposes unless a written objection is submitted prior to the course start date.

All course materials, including presentations and assessments, are the intellectual property of Provider and may not be reproduced, distributed, or posted without prior written permission except for Participant's personal educational use.

Liability, Indemnification and Termination

To the maximum extent permitted by law, Provider's liability for claims arising out of or related to this Agreement shall be limited to the amount of tuition actually paid by Participant for the specific activity. Participant agrees to indemnify and hold harmless Provider and its agents from claims arising from Participant's breach of this Agreement or negligent acts during the activity.

Provider may terminate this Agreement and remove Participant from the activity for cause, including a material breach of conduct or failure to comply with attendance and participation requirements. In the event of termination for cause, no refund shall be due except as required by applicable law.

By signing below Participant certifies that all information provided in this Enrollment Agreement is true and complete, that Participant meets eligibility requirements for the activity, and that Participant accepts and will comply with the policies, procedures and obligations set forth herein.

This Agreement shall be governed by the laws of the state or jurisdiction in which Provider maintains its principal place of business, without regard to conflict of law principles. Any dispute arising under this Agreement shall be resolved in the courts of that jurisdiction unless the parties agree in writing to an alternative dispute resolution process.
