Refund Policy: Participant acknowledges and agrees to the refund schedule and cancellation fees set forth by the Provider. Deposits are non-refundable unless otherwise agreed in writing by the Provider. Specific refund percentages are determined by notice period prior to start date and are incorporated into this Agreement.

Attendance, Assessments and Certification

Attendance Requirement: Participant must attend at least % of scheduled contact hours and complete required assessments to be eligible for certification.

Assessments and Practical Evaluations: Provider will administer written and/or practical assessments. Passing criteria will be communicated in program materials. Participant consents to evaluation and feedback as a condition of certification.

Materials, Intellectual Property and Recordings

Provider retains all intellectual property rights in curriculum, recordings, manuals and other training materials. Participant is granted a non-transferable license for personal use in connection with the program. Participant shall not reproduce, distribute, sell or publicly display materials without prior written consent.

Recording Consent: Participant consents    does not consent to audio/video recording of their participation. Provider may record sessions for training quality and assessment; recordings containing participant image or voice will be used only as permitted by consent.

Confidentiality, Liability and Indemnification

Confidential Information: Each party shall keep confidential all non-public information marked or reasonably understood as confidential disclosed in connection with the program. This obligation survives termination for a period of two years.

Liability: To the maximum extent permitted by law, Provider's liability for any claim arising out of or related to this Agreement is limited to direct damages not exceeding the tuition paid by Participant for the program. Provider is not liable for indirect, incidental, consequential or punitive damages.

Indemnification: Participant shall indemnify and hold harmless Provider and its agents against all claims, liabilities and expenses arising from Participant's breach of this Agreement, negligent acts, or unlawful conduct in connection with the program.

Health, Safety and Special Accommodations

Termination and Cancellation

Either party may terminate this Agreement for material breach if the breach is not cured within fourteen (14) days after written notice. Provider reserves the right to cancel a program for insufficient enrollment or unforeseen circumstances; in such case, Provider will offer refunds or credit toward a future program as specified in the refund policy.

Dispute Resolution and Governing Law

The parties shall attempt in good faith to resolve disputes arising under this Agreement. If unresolved, disputes will be resolved by binding arbitration in the jurisdiction specified by Provider or, if required by law, by courts located in the Provider's principal place of business. This Agreement is governed by the substantive laws of the state in which the Provider is located, without regard to conflict of law principles.

Acknowledgements and Participant Agreements

By signing this Agreement, Participant acknowledges that they have read, understand, and agree to comply with Provider policies, attendance and assessment requirements, and the refund policy. Participant certifies that the information provided in this form is true and accurate to the best of their knowledge.
