Assumption of Risk and Release: The signer acknowledges that participation in coaching activities involves inherent risks. The signer assumes all risks associated with participation and, to the fullest extent permitted by law, releases and holds harmless the coaching provider, its agents, employees, and contractors from all liability for personal injury, property damage, or other loss arising out of participation, except for intentional misconduct or gross negligence.

Indemnification: The signer agrees to indemnify, defend, and hold harmless the coaching provider from any claims, damages, losses, liabilities, and expenses (including reasonable attorney fees) arising from the signer's or applicant's negligence, breach of this agreement, or willful misconduct.

Emergency Medical Authorization: If in the opinion of the coaching staff the applicant requires immediate medical attention and a parent/guardian cannot be reached, the signer authorizes the coaching provider to obtain emergency medical treatment and agrees to be financially responsible for such treatment.

Photo/Video Release: The signer grants the coaching provider the right to photograph, record, and use images or recordings of the applicant for program promotion, educational materials, or archival purposes without compensation, unless the signer objects by checking the box below.

Cancellation and Refund Policy: Cancellations received in writing prior to the program start date may be eligible for a partial refund of tuition less any non-refundable deposit. After the program has commenced, refunds will be provided at the discretion of the coaching provider and may be prorated based on sessions attended. No refunds for voluntary early withdrawal without documented extenuating circumstances.

Confidentiality and Use of Materials: Coaching session content, proprietary materials, and individualized action plans are provided for the applicant's personal use. Unauthorized distribution or commercial use of materials is prohibited. The coaching provider reserves the right to terminate services for breaches of confidentiality or misuse of materials.
