This Coaching Enrollment Form (the "Agreement") records enrollment of the Student in the coaching services offered by the Provider. Submission of this form constitutes a binding agreement between the Provider and the Student or Parent/Guardian (if Student is a minor) upon acceptance by the Provider. Fees, schedules and program placement are subject to the terms set forth below.

Late Payment: Payments not received within 10 days of due date are subject to a late fee of $ and may be suspended from services until cured.

Refund Policy: Deposits are non-refundable except where required by law. Refunds for prepaid sessions may be issued on a prorated basis at the sole discretion of the Provider and less any administrative fees. Cancellations made at least 7 days prior to the first scheduled session are eligible for a refund of prepayments less deposit. No refund for no-shows.

Policies, Consent & Release

Attendance & Conduct: Student agrees to attend scheduled sessions on time and to comply with the Provider's code of conduct. Repeated cancellations or disruptive behavior may result in termination of enrollment without refund.

Medical Clearance & Emergency Treatment: Student (or Parent/Guardian) warrants that Student is physically able to participate in coaching activities. In the event of a medical emergency, the undersigned authorizes the Provider to obtain emergency medical treatment on behalf of the Student, and agrees to be responsible for any costs thereof.

Liability Release: To the fullest extent permitted by law, Student and Parent/Guardian release and hold harmless the Provider, its coaches, employees and agents from claims, liabilities or causes of action arising from Student's participation in coaching activities, including claims arising from ordinary negligence, except where caused by the Provider's willful misconduct or gross negligence.

Photo/Video Release: I grant the Provider the right to photograph and record Student during program activities and to use such images and recordings for promotional, educational, and administrative purposes. If I do not consent, I will indicate by checking the box below.
