COACHING PACKAGE AGREEMENT

This Coaching Package Agreement ("Agreement") is entered into on between Coach Name: and Client Name: .

Coach will provide coaching services as described below. Services are educational/coaching in nature and do not constitute psychological, medical, or legal advice.

Accepted payment methods:

Cancellation and refund policy: Client may cancel or reschedule sessions with at least notice. Deposits are unless otherwise agreed in writing. Partial refunds will be calculated pro rata for prepaid but unused sessions except where otherwise stated below.

Rescheduling & No-Show

If Client fails to attend a scheduled session without the required notice, the session will be charged in full. Coach will make reasonable efforts to reschedule missed sessions within the package period subject to availability. Extensions beyond the agreed end date require written agreement by both parties.

Confidentiality & Records

Coach will keep confidential all personal information and coaching session content, except as required by law or authorized in writing by Client. Notes and materials prepared by Coach remain the intellectual property of Coach; Client is granted a non-exclusive right to use materials provided for personal educational purposes only.

Client & Student Responsibilities

Client agrees to engage in the coaching process in good faith, complete assigned tasks, and notify Coach of any physical or mental health conditions, learning differences, or medications that may affect participation. Coach is not responsible for Client decisions or outcomes resulting from participation.

Termination

Either party may terminate this Agreement for material breach that is not cured within 14 days of written notice. Upon termination, unpaid fees for services already rendered are immediately due. Refunds for prepaid, unused sessions are subject to the refund policy described above.

Limitation of Liability & Indemnity

To the maximum extent permitted by law, Coach's liability for damages arising from this Agreement shall be limited to the amount of fees paid by Client for the coaching package. Client agrees to indemnify and hold Coach harmless from third-party claims arising from Client's use of coaching materials or failure to follow professional advice from other qualified practitioners.

Governing Law & Notices

This Agreement is governed by the laws of the jurisdiction in which Coach primarily practices. Notices must be given in writing to the addresses listed below and will be effective upon delivery.

Minor & Parent / Guardian Consent
