Notice of Rights and Election Procedures

You are being notified of your right to elect continuation of group health coverage under the Consolidated Omnibus Budget Reconciliation Act (COBRA). You have a limited period to elect continuation coverage. The election period is 60 days measured from the later of: (a) the date you receive this notice, or (b) the date coverage would otherwise end by reason of the qualifying event. If you elect continuation coverage, coverage will be effective retroactive to the date coverage terminated due to the qualifying event, provided the initial premium is paid as required.

Initial premium: per month, as determined by the plan. The first premium for retroactive coverage is due within 45 days after you elect continuation coverage, unless a different timeframe is specified in the plan documents. Subsequent premiums are due monthly and must be paid within the grace period described by the plan.

To elect continuation coverage, complete the election section below and return it to the plan administrator at:

Duration and Extensions of Coverage

Standard maximum period for continuation coverage is 18 months for most qualifying events. Coverage may extend to 36 months for certain events (death of the employee, divorce or legal separation, Medicare entitlement by the employee, or loss of dependent status). A qualified beneficiary determined to be disabled by the appropriate authority may be eligible for an additional 11-month extension (for a total of 29 months). Indicate applicable periods below.

Election of Continuation Coverage

To elect continuation coverage for yourself and/or your eligible dependents, check the election choice below and complete the coverage selection.

Rights, Obligations and Important Notices

This notice contains important information about your right to continuation coverage, the timeframes within which you must act to preserve coverage, and the conditions under which coverage may be terminated. You must notify the plan administrator of any address changes to ensure receipt of notices. Failure to pay premiums when due will cause termination of continuation coverage. The plan may terminate continuation coverage retroactively if full premium is not paid for the period of coverage.

The plan sponsor and plan administrator are responsible for providing accurate information regarding continuation coverage. This notice is provided to help you decide whether to elect continuation coverage; it is not a substitute for the plan’s written plan documents, which control the administration of benefits.

Legal Disclosures

At-Will Employment: Employment relationship, if any, is at-will, except where a written agreement provides otherwise. Either the employer or employee may terminate employment at any time, with or without cause or notice, subject to applicable law.

Equal Opportunity: The employer provides equal opportunity in employment and benefits without regard to race, color, religion, sex, national origin, age, disability, or other protected status as provided by law.

What the COBRA Health Insurance Notice Is and Why It Matters

COBRA Health Insurance Notice explains beneficiaries' rights to temporarily continue employer-sponsored group health coverage under the federal Consolidated Omnibus Budget Reconciliation Act (COBRA). When a qualifying event occurs — such as termination, reduced hours, divorce, or death of the covered employee — the plan must notify eligible individuals about their right to elect continuation coverage, applicable premiums, and election deadlines. The notice also identifies the plan administrator and gives step-by-step election instructions. Properly prepared and timely delivered notices preserve beneficiary rights and document employer compliance with COBRA and ERISA obligations.

Why a Compliant Notice Protects Beneficiaries and Employers

A clear, complete COBRA Health Insurance Notice preserves beneficiaries' access to temporary coverage, documents legal compliance, and reduces exposure to enforcement actions or litigation by demonstrating timely notice and accurate election instructions.

Why a Compliant Notice Protects Beneficiaries and Employers

  • Human resources and benefits administrators responsible for distribution, documentation, and audit trails.
