Late Payment Fee: Tuition past the Balance Due date is subject to a late fee and/or suspension from class as set by the Provider. The student agrees to pay any collection costs and reasonable attorney's fees incurred to collect unpaid balances.

Policies and Acknowledgments

Attendance and Make-up Policy: Regular attendance is required. Make-up classes may be permitted at the Provider's discretion and only where scheduling permits. Missed classes without prior notice may forfeit credit.

Code of Conduct: The student agrees to obey all safety instructions, kitchen rules, and hygiene requirements. Disruptive or unsafe behavior may result in removal from the class without refund.

Materials & Ingredients: Unless otherwise stated, ingredients and materials for demonstrations are provided. Students may be responsible for additional ingredient costs for take-home items. Provider is not liable for personal tools or equipment brought by students.

Health & Food Allergies Acknowledgment: I acknowledge that food prepared in class may contain allergens and that I have disclosed relevant allergies in the Medical / Allergies field above.

Photo/Video Release: I grant permission for the Provider to photograph or record my participation for promotional, instructional, or archival purposes unless I opt out by checking this box.

Release, Liability and Indemnification

Release of Liability: The student (and parent/guardian if the student is a minor) acknowledges that culinary instruction and activities involve inherent risks, including burns, cuts, slips, food-borne illness and allergic reactions. To the fullest extent permitted by law, the student releases the Provider, its officers, employees and agents from any liability for personal injury, property damage or other loss arising out of participation, except where due to the Provider's gross negligence or willful misconduct.

Indemnification: The student agrees to indemnify and hold harmless the Provider from any claims, damages, losses or expenses (including reasonable attorney's fees) arising from the student's actions or omissions during participation in the class.

Medical Authorization

In the event of a medical emergency, I authorize the Provider to obtain emergency medical care for the student. I understand I am responsible for all costs associated with such care.

Cancellation and Refunds

Deposit Policy: Deposits are required to reserve a place in class. Deposits are non-refundable except when the Provider cancels the class. The Provider will notify the student as soon as practicable if a class is cancelled.

Student Cancellation: Refunds for cancellations by the student are subject to the Provider's published schedule: full refund less deposit if cancelled more than 14 days prior to the start date; 50% refund if cancelled 7–14 days prior; no refund if cancelled less than 7 days prior, unless otherwise agreed in writing.

Privacy and Records

The Provider will maintain student records in accordance with applicable privacy laws. Personal information will be used only for administration of classes, safety, invoicing, and communications related to the educational services.
