By signing this Agreement, the Student (or Parent/Guardian on behalf of the Student) agrees to pay all tuition and fees assessed for enrollment in the course identified above. Tuition is due in accordance with the selected payment plan. The institution reserves the right to withhold grades, transcripts, and course access for accounts that are past due. Late payments shall incur interest and administrative fees as set forth below.

Refund and Withdrawal Policy

All requests for withdrawal or cancellation must be submitted in writing. Refunds, if any, will be calculated based on the date the written withdrawal is received by the institution and according to the institution's published refund schedule. The deposit/enrollment fee is non-refundable. A administrative processing fee may apply. The Student remains liable for all charges incurred prior to the date of withdrawal, including any unpaid tuition due under an installment agreement.

Emergency and Medical Information

Consent for emergency medical treatment: In the event of an emergency, if the Student is incapacitated and the Parent/Guardian cannot be reached, I authorize the institution or its agents to obtain necessary emergency medical treatment for the Student. This consent is limited to emergency action necessary to preserve life or prevent significant harm.

The institution maintains student records necessary for academic administration. The Student (or Parent/Guardian) may authorize release of academic records to third parties by submitting written authorization.
