Enrollment constitutes agreement to the Dance School Handbook, including but not limited to attendance expectations, punctuality, dress code, and conduct. The undersigned agrees that tuition payments are due according to the selected payment plan. Late payments may incur fees and may result in suspension of class participation until the account is current.

Refund Policy: Deposits are non-refundable. Refunds of tuition are granted only in accordance with written withdrawal submitted at least 30 days prior to the billing period or in the event of documented medical inability to participate; refunds will be prorated less the non-refundable deposit and any administrative fees. No refunds for missed classes due to student absence unless otherwise required by law.

Assumption of Risk and Release: Dance activities involve physical exertion and inherent risk of injury. The undersigned hereby acknowledges these risks and releases and agrees to indemnify and hold harmless the Dance School, its instructors, employees, and agents from any and all claims, liabilities, losses, costs, or expenses arising from participation, except to the extent such claims arise from the Dance School's gross negligence or willful misconduct.

Medical Authorization: In the event of an emergency, if the undersigned cannot be reached, the Dance School is authorized to seek and consent to medical treatment for the student. The undersigned agrees to be financially responsible for any medical expenses incurred on the student's behalf.

Photo / Video Release:

Behavioral Standards: The undersigned agrees that inappropriate behavior, bullying, or disruptive conduct may result in dismissal from classes without refund. The Dance School reserves the right to remove any student whose conduct is detrimental to the welfare of others.

COVID-19 / Communicable Disease Acknowledgment: The undersigned will notify the Dance School if the student has been diagnosed with or exposed to a communicable disease. The Dance School may modify class delivery or require testing or isolation in accordance with public health guidance and internal policy.
