A. Assumption of Risk: I acknowledge that scuba diving and associated training activities involve inherent risks including but not limited to drowning, decompression sickness, barotrauma, equipment failure, hypothermia, and other personal injury or death. I understand that these risks cannot be eliminated without jeopardizing the essential qualities of the activity.

B. Medical Fitness and Clearance: I certify that I am medically fit to participate in diving instruction. I agree to disclose all medical conditions and medications. If medical conditions exist that could affect diving fitness, I will obtain written clearance from a licensed physician prior to participation. The training provider may require medical clearance as a condition of participation.

C. Release and Indemnity: In consideration of being permitted to enroll and participate, I hereby release, waive, discharge and covenant not to sue the training provider, its instructors, agents, employees and contractors from liability for any loss, damage, injury or expense, including death, arising out of my participation, whether caused by negligence of the provider or otherwise. I agree to indemnify and hold harmless the training provider from any claims, demands or actions brought by me or on my behalf.

D. Refund and Cancellation Policy: Deposits are non-refundable except as required by applicable law. Refunds for cancelled courses will be provided in accordance with the training provider's standard policy; fees for materials, pool time and instructor services may be deducted. The provider may cancel or reschedule courses for insufficient enrollment or unsafe conditions and will provide notice and refund or reschedule options.

E. Photography and Media Consent: I consent to the use of my image or likeness in photographs or video taken during training for training records, safety documentation, or promotional materials unless I indicate otherwise in writing prior to participation.
