Attendance and Conduct: I acknowledge that regular attendance at rehearsals and performances is a condition of participation. I agree that the student will comply with the Drama Club code of conduct, follow directions of staff and volunteers, and accept reasonable disciplinary measures for violations. I understand that excessive unexcused absences may result in removal from a production.

Liability Waiver and Medical Authorization: In consideration for participation, I release and hold harmless the school/organization, its employees, volunteers and agents from liability for claims arising from ordinary negligence. In the event of injury or illness, I authorize emergency medical treatment as deemed necessary by authorized personnel and consent to transport to an appropriate medical facility.
