EDA TENNIS COACHING AGREEMENT

Cancellation by Client: Client must provide at least notice for cancellation. Late cancellations or no-shows are charged in full unless Coach cancels or reschedules.

Make-up Sessions: Make-ups are offered at Coach's discretion and must be scheduled within of missed session. Unused make-ups are forfeited at program end.

Health, Medical and Emergency

Authorization for Emergency Treatment: In the event of medical emergency, I authorize Coach to seek emergency medical treatment for the Student if I cannot be reached. I agree to be financially responsible for such treatment.

Release, Waiver and Indemnification

Assumption of Risk and Release: I acknowledge that participation in tennis training involves physical activity and inherent risks including injury. I voluntarily accept those risks on behalf of the Student and agree that Coach, its employees, agents, and assigns shall not be liable for claims, demands, or causes of action arising from injury, death or property loss unless such injury is caused by gross negligence or willful misconduct by Coach.

Indemnification: Client agrees to indemnify and hold harmless Coach from and against any and all claims, suits, liabilities, losses, damages, expenses and costs (including reasonable attorneys' fees) arising out of Student's acts or omissions except to the extent caused by Coach's gross negligence or willful misconduct.

Photo/Video Release

I authorize Coach to photograph and/or video the Student during training and events, and grant Coach a perpetual, royalty-free right to use such media for promotional, educational or training purposes. Decline: Accept:

Code of Conduct

The Student and Parent/Guardian agree to respect Coaches, staff, facilities, other players and follow instructions. Unsportsmanlike conduct or repeated violations may result in suspension or termination of training without refund. Initial to acknowledge Code of Conduct:

Termination and Governing Law

Termination: Either party may terminate this Agreement with written notice. Termination by Client does not relieve Client of payment obligations for services already rendered or reserved unless otherwise agreed in writing.
