I acknowledge that participation in the activity described above involves inherent risks, including but not limited to physical injury, illness, property loss, or death. Such risks may arise from travel, equipment, natural conditions, participant actions, or the actions or negligence of others. By signing this form I voluntarily assume all such risks, whether known or unknown, foreseeable or unforeseeable.

I hereby release, waive, discharge, and covenant not to sue the school, its trustees, employees, volunteers, agents, and affiliates (collectively referred to as the Institution) from any and all liability, claims, demands, actions, or causes of action arising out of or relating to any loss, damage, injury, or death that may be sustained by the student while participating in the activity, except to the extent such loss is caused by the gross negligence or willful misconduct of the Institution.

I agree to indemnify and hold harmless the Institution from any claims, causes of action, losses, damages, or liabilities, including attorneys' fees and costs, arising out of the student's participation, except where such claims are the result of the Institution's gross negligence or willful misconduct.
