Assumption of Risk: The participant acknowledges that participation in the boot camp involves physical, academic, and other risks. The participant voluntarily assumes all risks associated with participation and releases the Program and its staff from liability for injury, loss, or damage arising from participation except to the extent caused by willful misconduct or gross negligence of the Program.

Emergency Medical Authorization: In the event of an emergency, the undersigned authorizes program personnel to secure emergency medical treatment for the participant and to consent to any necessary medical treatment, including hospitalization. The undersigned agrees to be financially responsible for costs incurred as a result of such treatment.

Indemnification: The undersigned agrees to indemnify and hold harmless the Program, its officers, employees, agents, and volunteers from any and all claims, demands, actions, or causes of action arising out of the participant’s participation, except for claims arising from the Program’s gross negligence or willful misconduct.

Photo/Video Release: The participant grants permission for the Program to photograph and record the participant during program activities and to use such photographs and recordings for promotional and educational purposes without compensation, unless the box below is checked to withhold permission.

Behavior, Attendance & Refund Policy

Conduct: Participants must comply with the Program code of conduct. The Program reserves the right to dismiss any participant whose behavior endangers others or materially interferes with program operations. Dismissal for misconduct will not entitle the participant to a refund.

Cancellation & Refunds: Refunds are available only as set forth in the Program’s published refund policy. If the participant withdraws prior to the start date, a partial refund may be granted less administrative fees. No refunds will be issued for voluntary withdrawal after the program begins except in extraordinary circumstances determined by the Program director.

Privacy & Use of Information

The Program collects and maintains participant information for purposes of safety, communication, program administration, and required reporting. The Program will not disclose personally identifying information except as necessary for medical treatment, legal compliance, or with consent of the parent/guardian for minors.
