Employer agrees to maintain and produce, upon request, payroll records, time sheets, and other documentation necessary to verify incentive eligibility and amounts. Employer agrees to repay any incentive funds that were disbursed in error or as a result of misrepresentation within 30 days of written demand.

Employer acknowledges that payment is contingent upon program verification and available funding. The educational institution reserves the right to audit records related to this incentive for a period of three years from the date of payment. Employer is responsible for all local, state, and federal tax reporting related to incentive payments and employment of student(s). Employer agrees to comply with all applicable labor, workplace safety, nondiscrimination, and immigration laws.

Employer further agrees to indemnify and hold harmless the educational institution from any claim, liability, or expense arising out of Employer's failure to comply with the terms of this form or applicable law.
